puring normal operation, an operator found the valve stem on the HPCI stop valve l

broken separating the valve from the operator. The redundant sy. stem, ADS, was oper-t, i.; able as were all low pressure injection systems. Since the valve is pushed open bv 1 the operator, the HPCI would have cerformed its intended safety function if recuired!.
