While performino Excess Flow Check Valve Surveillance. crecadura OfS 67-1 chect

, j salves 1-263-2-318 and 1-263-42A failed to check the flow throuch the instrument i

l lines.

Each line has a nanual isolation valve uostream from the flow check valves I

i which could have isolated the flow i f the need arose.

The failure of the check

[

i valves to limit instrument line flow did not hinder the operation of the associated a

enhanced by low flow conditions and by the fact that these valves do not chance position d'uring normal plant operation. The valve internals were removed, cleaner' l

The valves-ware then successfully tested. A previous failure of thij g

type occurred on March 5, 1978, and was reported in LER/R0 78-14/03L.
