It was necessary to isolate this piping so that a system =odification could be =ade to allev filling the syste= piping with an anti-freece solutien to prevent system freese-up in the Diesel Generater Radiater Roc =. About 5h hours passed until a centinuous fire watch "ns estab-lished in accordance with Technical Specification 318.3 This incident was the result of a misunderstanding by the shift superviser cf the recently issued Fire Protection Technical Specifications.

On January lk, 1978, approxi=ately 18 hours2.083333e-4 days <br />0.005 hours <br />2.97619e-5 weeks <br />6.849e-6 months <br /> after FS-V156 had been opened and the continuous fire watch secured, it was discovered that an isolation va'.ve that had been installed as part of the modificatien was closed, isolating the fire sprinkler system piping to the "3" Inergency Diesel Generator Radiater Recm.

Upon this discovery, a continuous watch was established until the valve was opened.

This incident was the result of a failure to note that although FS-7156 had been opened allowing flov to the Energency Diesel Generater Room, the isolation valve installed as part of the =cdification had not been opened.

Continuous surveillance of the "3" Diesel Generator and Radiator Eccms during periods of "B" Diesel Generator operation and periodic inspection of the Diesel Generator and Radiator Roems provided adequate fire protection, and no threat to the health and safety of the ;ublic resulted.

In both incidents, a continuous fire watch was established i==ediately upon determination of the violations.

The shift supe.rvisors, shift fore =en, and their operaters shall review the requirements of the recently issued Fire Protection Technical Specification.

Since the Fire Service System is within the secpe of the Quality Assurance Fregrs=,

precedural guidance which shculd prevent recurrence of the above events is pro-vided in Ad=inistrative Procedure AP 1001.
