43 44 47 CAUSE DESCRIPTION AND CORRECTIVE ACTION 0 i O l The cause of the inoperability was failure of an operational amplifier (Struthers - l Fi TTl i Dunn, Inc. , Model 12A) in the alarm card which monitors sensor signal level and g,i,i l alarms on sensor failure. The op-amp was replaced, the hydrogen analyzer surveil- l 3 3 l lance was complete' satisfactorily and the unit returned to service. l 2 4 I I 7 8 9 80 ST S  % PO'.*JE R OTHER STATUS blSCOV Y DISCOVE HY DESCRIP TION i s lE l@ l 1 l 0 l 0 l@l t1A l lBl@l Surveillance Test l A TIVIT / CO TENT RELEASED OF RELF ASE AMOUNT OF ACTIVITY LOCATION OF REL EASE l1168 9l l Zl @ Wgl 7 10 tlA 44 l l 45 t1A 80 l
