CAUSE DESCRIPTION AND CORRECTIVE ACTIONS h i o l The cause of this occurrence was due to a broken bolt in the valve motor ooerator. l i , l The broken bolt caused the motor stator to be out of alignment, and rub acainst the _j rotor. Excessive motor current caused the breaker to trip . Corrective action was l l to replace all four bolts on the motor operator and successfully retest the valve for l
