During normal operation, Operations personnel noted improper inputs at the "A" and "C" levels to Rod Block Monitor "A".

The instrument was declared in-operable and the redundant channel (Rod Block Monitor "B") was verified operable by completing Operations Surveillance Test F-ST-5R ti tled " Rod Block Moni tor Upscale and Downscale Instrument Functional Test" to satisfy the requi rements of Technical Speci fications Appendix A, Table 3.2-2.

Since the redundant instru-ment was fully operable the event did not represent any significant hazard to the public health and safety.

Investigation revealed a failed integrated circuit. The circuit was re-placed and the instrument was recalibrated to verify proper operation and returned to service.
