Event Date: 4/7 /78 Temporary Loss of Flow Path from the Boric Acid Tanks
- 1. Description of Event:
During normal station operation at rated power, the operator observed an increase in level of A Boric Acid Storage Tank (BAST). A corresponding drop was noted in B BAST level. Another operator inspecting for the cause of the problem discovered a pencil stream leak_ from the body-to-bonnet joint of l-CH-98, a cross connect valve between the discharge lines of A and B Boric Acid Transfer pumps. Leak isolation and repair required shutdown of
.both transfer pumps. Since A pump was supplying. one boron flow path to the
- core, pump shutdown was contrary to Technical Specification 3.2:B.4 and was reportable in acco-rdance with_ Technical Specification 6 .6. 2. b(.2).
- 2. Probable Consequence of Event/Status of Redundant Systems:
The temporary stopping of the Boric Acid Transfer pumps had no effect upon the health or safety of the general public because in the event of a safety injection, the charging pumps take suction from the refueling water storage tank. This second path was operable.
- 3. Cause of Event:.
The temporary stopping of "A" Boric Acid Transfer pump was prompted by failure of valve l-CH-98. The rupturing of the valve's diaphragm permitted the transfer of Boric Acid from Tank B to A, and-leakage to the floor of the Auxiliary Building.
- 4. Immediate Corrective Action:
The immediate action was to commence unit ramp down. Valve 1-CH-98 was then repaired by replacing the ruptured diaphragm. The valve was returned to service and "A" transfer pump restarted. With two flow paths, as required, the Unit was returned to full power.
- 5. Scheduled Corrective Action No other corrective action is scheduled.
- 6. Action Taken to Prevent Recurrence Replacement of the failed valve's diaphragm suf:fices as an action to prevent recurrence.
- 7. Generic Implications At this time, the failure of the diaphragm valve is not considered to have generic implications, since these valves are covered by a periodic dia-
- phragm replacement program. This failure was a random failure within that program.
