Before sending a request for additional information via email, I called Ms. Maria Kampsen to go over my proposed questions and request for additional information. One of the questions was "please indicate if your company (Alaska Testlab, LLC) has or ever had an NRC or Agreement State license for portable gauges. If so, please state the NRC or Agreement State license number." Ms. Kampsen wasn't sure of the date or year but she thought the license was in the name of Alaska Testlab at some point in time. I agreed to do a search in ADAMS on Dow! LLC dba Dow! HKM License# 50-21252-01; Docket# 030-20103 to see ifl could find where this license number was ever issued to the entity of Alaska Testlab. The license renewal dated June IO, 2003 (amendment# 9) for license#
50-21252-01 was issued to Dow!, LLC dba Alaska Testlab. In addition, a corrected copy to amendment# 9 to this license renewal was issued on August 20, 2008 to the same licensed entity of Dow!, LLC dba Alaska Testlab.
Therefore, based on the information above, Step 1 - Initial Screening of the Checklist to Provide a Basis for Confidence that Radioactive Materials will be Used as Specified on the License, will be "Y" indicating that the applicant is a known entity.
