Ouring normal full power'operation .the 1B diesel generator (0/G) output breaker inadvertently shut during breaker maintenance with the 0/G secured. The breaker's control power had been removed for the maintenance, but the breaker shut unexpectedly as it was being manually racked out. The ensuing electrical transient resulted in a loss of the HBH side electrical system and a reactor trip on the low S/G water level.

The HBH side electrical system was repowered from its startup transformer and plant conditions were stabilized. The 0/G output breaker was replaced and the 0/G retested satisfactorily. The cause of the 0/G breaker closure was binding in the MOC switch rod assembly. The health and safety of the general public was not affected by this event.

Hand operated pump aad plastic bucket - both marked "FOR DIESEL ENGINE LUBE OIL SAMPLING ONLY" located in cold tool room, indelible pen, rags 3.2 Three (3) 500 ML bottles (M&S 0 027-73000-0), available from PM Planner. If drawn from Stores, wash out bottles with isopropyl alcohol and allow to air dry. Mark two (2) of the bottles to identify the

engines (2A1 & 2A2) from which the samples are to be taken, and date.

The third bottle is for purginp the pump.

4. Instructions:

4.1 Cleanliness is of utmost importance. Care should be taken aot to contaminate bottles or hose ends while taking samples.

4.2 Remove clips from pump suction and discharge hoses and wipe off outside of hoses 4.3 Remove the diesel engine lube oil dip stick from 2A1 engine aad insert suction hose of hand pump into dip stick tube, being careful not to touch the bottom of the sump with the hose.

4.4 Prior to procuring the sample, purge the pump of residual oil by filliag the unmarked bottle with lube oil from the sump. Ib not allow the discharge hose to enter the bottle. After purge bottle is filled, discard its contents into the bucket.

4.5 After purging, sump.

fill the'bottle marked 2A'1 with lube oil from the engine Ib not allow the discharge hose to enter the bottle. Cap the bottle after filling.

4 6 Remove hand pump suction hose from dip stick tube and pump remaining oil in pump into bucket.

4.7 Repeat sequence on 2A2 engine.

4.8 Upon completion of sampling, place clips on pump hose eads.

4.9 Take samples to PM Planner.

3. Instructions: (continued) 3.3 (continued) 3.3.4 Remove bolts from the three (3) eight (8) bolt flanges which hold the temperature regulating valve (thermostat). The housing which holds the thermostat is stamped AMOT. (12 CYL only) 3.3.5 To remove the housing, the flexible spool piece may also have to be removed in order to get the housing out of the saddle mounting. (12 CYL,only)

NOTE Remove bottom plate on AMOT to remove thermostat 16 CYL only.

3-3.6 After AMOT housing is removed, see disassembly and inspection, Pages 2 and 3, MI 581.

3.3.7 Replace ~ings 12 cyl. M&S 8574-00572, 16 cyl. M&S 8574-00576 3.3.8 If necessary,'replace upper seat assembly See Page 3 of MI 581 3.4 Remove suction flanges on the water pumps and inspect the pumps.

Install new gaskets. 12 cyl. M&S 8651-29402-7, 16 cyl. M&S 651-29702-6 See Tech. Manual Section 810 3.5 Fill cooling system to bottom of sight glass in expansion tank. Have Chemistry Department 'add Nalcool 2000 to system.- Check for leaks 3.6 Check overspeed trip setting. See Tech.'anual Section 813.

3.7 Drain lube oil into clean drums. See Tech. Manual Section 89.

3.8 Inspect for fuel oil leaks into lube oil system and repair as necessary..

See Tech. Manual Section 89.

3.9 Replace soak back pump oil filters M&S 8651-12502-1 3.10 Clean lube oil strainers and screens. Change lube oil filters and turbocharger oil filter.

3.10 1 Remove L.O. strainer from housing and clean in accordance with Section 9, Pages'-6 through 9-8 of Maintenance Manual 2998-7434.

L.O. strainer gasket kit M&S 8651-34802M 3 10.2 Check seal rings and gaskets for any damage. Replace if necessary.

3 10 3 Replace turbocharger oil filter M&S 8650-83102-9. See Tech. Manual f2998-7434, Section 9, Pages 9-1.5 and 9-16.

3.10.4 Change engine lube oil filters See Section MI 926, Pages 1, 3 and 4 of Tech. Manual 2998-7434. L.O. filters M&S 8651-12502-1 (Qty.-7).

3.11.1 disconnect the air line at the ejector. See location picture.

3.11.2 Remove the bolts from separator cover.

3.11.3 U'nbolt the ejector assembly at the flexible connection and remove the separator cover and ejector as an assembly.

3.11.4 Remove the screen element assembly and wash it in a petroleum

-solvent. Riase. -the element with hot water and blow -dry with-air.

3.11.5 Replace the element and the cover and ejector assembly.

3.11.6 Connect air line to effector.

3.12 Check fuel rack settings and injector timing settings. Adjust as necessary.

3.13.Visually inspect all cylinder head accessories for tightness and damage.

'3 14 Take piston clearance to cylinder head readings on all cylinders. See Tech. Manual Section 5, Page 5-19, Steps 61A through 61F.

3.15 Inspect pistons and condition of compression rings. See Tech. Manual Section 3, P 3 2 and 3.3.

3 16 Inspect'ylinder liners. See Tech. Manual Section 4, P. 4-l,.

3 '7 Inspect and replace as necessary all engine inspection cover gaskets HGS 8651-27602-9.

3.18 Remove turbocharger inlet and exhaust connections. Remove compressor shroud and inspect compressor wheel for nicks, shaft end float, etc.

Inspect exhaust side of turbocharger for damage.

3.19 Replace turbocharger air filters.

3.19.1 To replace the turbocharger air intake filters, remove front cover and pull filter elements, one at a time, from the frame. Filter M6S %65-1000.

3.20 Inspect fuel system for leaks. Repair as necessary.

3.21 Clean the fuel suction strainers. Strainers are located on accessory sk'd at fan end of engine 3.21.1 Move the strainer selector lever to the opposite strainer of the one being serviced.

3.21.2 Unbolt and remove fuel oil strainer cover. Remove drain plug and drain fuel oil into bucket(s).

3 21.5 Reinstall strainer element. The MGS number for a new strainer element is 065W3080 if required 3 21.6 Make a new housing to cover gasket (Garlock) and install. Measure thickness of old gasket before discarding.

3.21.7 Replace cover and tighten bolts<.

3.21.8 Remove 1/8" pipe plug vent. Prime the strainer by operating the manual fuel priming pump (located near the strainer, see attached drawing) until fuel starts to flow out of the vent hole.

3.22.4 Unscrew the filter to be changed and discard.

3225 Apply a film of oil to the gasket of the new filter and screw it on by hand but do not tighten; leave a small gap between the filter and cover. Prime the filter by operating the manual fuel priming pump until fuel starts to,flow out of the top .of the filter F.O.

3.4 Remove suction flanges on the water pumps and inspect the pumps.

See Tech. Manual Section 810 3.5 Fill cooling system to bottom of sight glass in expansion tank. Have Chemistry Department add Nalcool 2000 to system. Check for leaks.

3.6 Check overspeed trip'etting. See Tech. Manual Section 813..

3.7 Drain lube oil into clean drums. See Tech. Manual Section 89.

3.8 Inspect for fuel oil leaks into lube oQ. system and repair as necessary. See Tech. Manual Section 89 3.9 Replace soak back pump oil filters M&S 8651-12502-1.

3 10 Clean lube oil strainers.'and screens. Change lube oil filters and

. turbocharger oil filter.

3 10.1 Remove L.O. strainer from housing and clean in accordance with Section 9, Pages 9-6 through 9-8 of Maintenance Manual 2998-7434. L.O. strainer gasket kit M&S 8651-34802-0.

3.10.2 Check seal rings and gaskets for any damage. Replace if necessary.

3.10.3 Replace turbocharger oil,filter M&S 8650-83102-9. See Tech.

Manual 82998-7434, Section 9, Pages 9-15 and 9-16.

1. Safety Precautions:

1.1 Be alert diesels may start unexpectedly.

2. Equipment Needed:

2.1 Clipboard, pen, and flashlight.

3. 'nstructions:

3.1 Foreman will contact Operations to have a N.O. avaQ.able to work with a mechanic for the purpose of verifying starter motor Bendix Gear/Flywheel engagement.

3.2 The Operator will be responsible for the proper valve lineup so that only one set of starter motors will be operable and with the other three sets inoperable during the test. (A set consists of two starter motors.)

3.3 With the mechanic in position to observe the two operable starters, the operator, at the mechanics signal, will activate the starting system mo entarily to allow the Bendix gears to engage and attempt to turn the flywheel. As soon as the flywheel starts to rotate, the operator is to cease activation of the starting system.. Verification of this test will be recorded by the mechanic on page two of this PM.

3.4 Repeat steps 3.2 and 3.3 above until all four sets of starters have been tested.

1.1.2 Ensure air receiver outlet valves are closed and the air line drain valves located under the deck plate between air receiver and diesel engine are opened.

1.1.3 Do not remove aay .tools, supplies, or trash from RCA until cleared by HP.

2.1 "A" Diesel Generator Building'ools 30 and Supplies Needed:

3.1 Mechanics Tool Box, 2-3/8" combination wrench, small wire brush, small, fuanel, face shield.

3 2 One quart of Regal R & 0 32 oil, kerosene and rags.

4. Instructions:

4.1 Air Line Lubricators:

4.1.1 Verify oil level in lubricator bowl. Add Regal R & 0 32 if needed.

4.1.2 Check needle valve, ensure that valve 'is not seated and is backed off l-l/2 turns 'for proper OG. flow.

4 1 3 To ensure that the lubricators are supplying oil to the starters, check the>> draia hole in the box the exhaust hoses discharge into, for oil residue. If the lubricator's are not functioning properly, remove the top plug and drip gland. Clean parts and passages, using kerosene and blow out with compressed air.
