Owner's Activity Report Submittal of Fifth 10-Year Interval 2015 Refueling Outage Activities, SVP-15-046, Dated June 19, 2015 Enclosed is the Quad Cities Station Owner's Activity Report Submittal of Fifth 10-Year Interval 2015 Refueling Outage Activities, SVP-15-046, Dated June 19, 2015.

This report was originally mailed via US P.O to NRC on June 19, 2015. Quad Cities Station received today, the original mailing envelope (with original contents somewhat damaged) as returned with an envelope note/stamp stating, "RETURN TO SENDER, INSUFFICIENT ADDRESS, UNABLE TO FORWARD, RETURN TO SENDER." The original mailing envelope was damaged and appeared to have been heated since the original contents text had been blurred and also appeared on the backside of the letter. As such, the original letter of June 19, 2015 is now enclosed and submitted to NRC Document Control Desk again. The damaged envelope and stamp have been shown to the Station NRC Resident Inspector as evidence of damage and being returned to sender.
