Davis-Besse Nuclear Power Station, Unit 1 Docket Number 50-346, License Number NPF-3 Readiness for NRC Supplemental Inspection Required for a White Performance Indicator In Integrated Inspection Report 05000346/2016001 and Assessment Followup Letter dated April 26, 2016, the Nuclear Regulatory Commission (NRC) documented Davis-Besse Nuclear Power Station (DBNPS) performance indicator for Unplanned SCRAMS with Complications had crossed the green-to-white threshold, effective the end of the first quarter of 2016. This letter also documented the NRC's assessment that the performance of the DBNPS remained in the Regulatory Response Column of the Reactor Oversight Process Action Matrix due to the addition of this White input for the performance indicator. This was in addition to an unrelated greater-than-green Security Cornerstone input that has since been closed as documented in Supplemental Inspection Report 05000346/2016408 and Assessment Follow-Up Letter dated August 10, 2016. As requested by the April 26, 2016, letter, this letter is to inform the NRC staff that FirstEnergy Nuclear Operating Company (FENOC) is ready for the required supplemental inspection at the DBNPS for the white Unplanned SCRAMS with Complications performance indicator in accordance with Inspection Procedure 95001.

Each of the three Unplanned SCRAMS with Complications events affecting the performance indicator were entered into the site Corrective Action Program and evaluated at the Root Cause evaluation level. Following the investigation of the individual events, an aggregate review of these three events was then performed by a multi-discipline team, and used techniques required by its Root Cause classification.

The aggregate review included an extent of condition and extent of cause review, along with an evaluation as to whether safety culture components were significant contributors. Corrective actions were developed by the aggregate review in conjunction with the corrective actions from the three individual events to address the root and contributing causes to prevent recurrence of Unplanned SCRAMS with Complications that resulted in the white performance indicator.
