1. The Discharge Monitonng Report (DMR) is required to be sent to the Pennsylvania Department of Environmental Protection (DEP) per NPDES Permit No. PA0025615.

A copy of the letter and the reports are forwarded to the US EPA (also required by regulations and the Permit) and the US NRC (current expectation of the NRC).

2. The report due date (postmark) is the 28h of the month.
  • Signatures indicate that the submittal was reviewed for accuracy and completeness, and that no significant information has been excluded from the submittal, such that, due to its absence, the reader would be materially misled.
    • Additional signatures may not be required from the Section/Department if responses are provided in writing and are signed by the Section/Department managers for providing input into the submittal. These documents should accompany the submittal when routing for final approvals.

BLOCK 2 LETTER SUBJECT - Initiator enters the subject as written on the subject line of the letter.

BLOCK 3 PREPARED BY - Initiator enters the names of individuals responsible for providing technical information for the correspondence along with his/her name.

BLOCK 4 DATE RESPONSE DUE TO THE NRC - Initiator enters the date the correspondence is due to the NRC. If the correspondence does not have a required due date, the block shall be marked not applicable (NA).

BLOCK 5 POSTING REQUIRED BY 10 CFR 19.11 - Initiator checks if the letter is required to be posted per the requirements of 10 CFR 19.11.

BLOCK 6 COMMITMENTS CONTAINED IN LETTER - Initiator enters either Yes or No based upon the content of the letter. Commitments are to be summarized and included in an attachment to the letter or, if no commitments are being made, include statement to that effect in the letter.

BLOCK 7 NOTARY REQUIRED - Initiator checks if the correspondence is required to be notarized or if correspondence provides affirmation, as required.
