Instructions for Completing Checklist, This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.Anchorage 1. Is the anchorage configuration verification required (i.e., is the item one .Y[ NO of the 50% of SWEL items requiring such verification)?

The item is one of the 50% of SWEL items requiring anchorage configuration verification.

2. Is the anchorage free of bent, broken, missing or loose hardware? The anchorage for the pump is free of bent, broken or loose hardware;however, this is not the governing component for possible anchorage failure. The six (6), 3/4" CIP bolts attaching the skid to the concrete footing is the governing anchorage for potential failure based on the SEWS evaluation and has been determined to be free of bent, broken, missing or loose hardware.

There are two bolts utilized for guiding the motor into place onto the skid, these two bolts where never removed and have loose nuts. However, this is not an issue since these bolts are not utilized to provide anchorage capacity for the equipment.

This is more of a housekeeping issue; it is recommended to remove the nuts to stay consistent with plant documentation.

This is not an adverse seismic condition.

3. Is the anchorage free of corrosion that is more than mild surface oxidation?

Anchorage is free of corrosion that is more than mild surface oxidation.

The SLC temperature element sensor wire (Equipment 11 TE-49) is located near an unanchored Unistrut support. The unanchored Unistrut support can slip from the base and potentially damage the sensor wire during a seismic event; however, this is not considered as a seismic adverse interaction.

Due to uneven surface on the concrete footing there is a gap between the anchor plate and the concrete footing. Gaps on the ends of the anchor base plate are close to 0.5" Gaps between the anchor base plate and the concrete footing at the anchor locations is less than a 1/4" which by engineering judgment is not large enough to cause a significant moment on the bolts.Evaluated by: Harpreet Ghumanb A Yaroslav Losev Date: 09/25/2012 09/25/2012 Attachment C Engineering Report No. JAF-RPT-12-00015 Rev. 0 Page 90 of 573 ATTACHMENT 9.6 SEISMIC WALKDOWN CHECKLIST FORM Sheet 4 of 4 Status: YE NE U-I Seismic Walkdown Checklist (SWC) SWELl-161 Equipment ID No. 11TK-1 Equip. There is a 1" copper line attached to the SWEL item that is less than 1/8" of clearance between a handrail and a P1000 UNISTRUT.However, this could be considered beneficial, since at the points for which the copper line may come into contact with the handrails during a seismic event, the handrails will provide support in one direction which will restrict the deflection of the copper line and reduce stress on supports on the same line.Comments (Additional pages may be added as necessary)

At the front support of the condenser, Four 5/8" DIA HKB-3 X 10" Long anchors are used to install the 1" THK base plate and the Y," THK mounting plates. The 5/8' vendor provided plate is welded on top of the mounting plates, making this connection rigid. At the back support, a sole plate 3/4"OX 2' X 6'A36, is installed on top of the existing concrete pedestal.Eight 5/8" DIA. Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each'of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.Anchorage 1. Is the anchorage configuration verification required (i.e., is the item one -Y[ NEI of the 50% of SWEL items requiring such verification)?

2. Is the anchorage free of bent, broken, missing or loose hardware? No bent, broken, loose or missing hardware.3. Is the anchorage free of corrosion that is more than mild surface oxidation?

Mild surface rust on painted anchorage brackets 4. Is the anchorage free of visible cracks in the concrete near the anchors? No concrete in area YO N[l U[:I] N/A[-]Y[ N[-1 U[I N/A[--YE] NIl! UOL N/A[1 Enter the equipment class name from Appendix;B:

The anchorage configuration matches the descriptions on SEWS for 71BMCC-1 6. Based on the above anchorage evaluations, is the anchorage free of YO NEI UE-potentially adverse seismic conditions?

Interaction Effects 7. Are soft targets free from impact by nearby equipment or structures?

The equipment is free of soft targets. The only external devices to the panel doors are robust switches.8. Are overhead equipment, distribution systems, ceiling tiles and lighting, and masonry block walls not likely to collapse onto the equipment?
