The RRI reviewed commitments listed in HL&P letter (ST-HL-AE-533), dated September 18, 1980. Based on the review of objective evidence and actions taken, the following item is closed.

Basis for Closing A8 The commitment stated that B&R QA Engineers are responsible for design reviews, purchase order reviews, nonconfomance report reviews and preparation of in-spection checklists.
The NRC inspector interviewed eight (8) B&R QAE Managers / Supervisors to determine that this commit-ment was implemented. Five (5) personnel, at the working level, were also interviewed. The personnel who were contacted provided several examples which showed that reviews were in progress.
Additionally, the NRC inspector reviewed a file con-taining objective evidence of reviews. A log, dating back to December 1980, documented review of procedures, design documents, purchase orders and preparation of inspection checklists. Examples of such reviews were attached.
The RRI reviewed the following areas and determined that corrective action was incomplete and further review is necessary.

The B&R QA Site Organization, as described in the Licensee's Response to the Show Cause Order, was composed of Quality Engineering, Quality Control, Vendor Surveillance (off site), QA Services and Audits.
In August -Sept-ember 1980, this organization was slightly modified and was subsequently documented in t'n Revised QA Program Description transmitted by HL&P letter (ST-HL-AE-568), dated October 31, 1980 The NRC inspector reviewed the staffing level for these organizations and l
on November 18, 1980 was infomed that the Site Surveillance Group would

be composed of fourteen (14) people. On April 29, 1981, the inspector found that the Internal Surveillance and Subcontracts Surveillance Mech-

l anical Sections were not fully staffed, as stated in November 1980.
Neither the QA Procedure ST-QAP-1.1, Organization, nor ST-QAP-18.2, Site

Surveillance Program, a.dequately or fully describes the areas of respon-sibility and duties of the four surveillance groups. Also the basis for determining the frequency for perfonning surveillances is unclear. As a result of this review, and subsequent discussions, the licensee is to meet with the contractor to assess implementation in this area.
This matter is an unresolved item pending further review.

(Closed) Show Cause Order, Item V.A.(6): The Licensee Shall Develop and Implement a More Effective System to Provide for the Identification and Correction of the Root Causes of the Nonconformances which occur.
The RRI reviewed omitments listed in the previously referenced letter (ST-HL-AE-533), and objective evidence of implementation. The following items are closed.

Basis for Closing M31 The commitment stated, " Attempt to expedite ANI Review of QA Manual will be made".
The RRI reviewed a series of 3&R letters as follows:
ST-BR-HB-44, 412, 41, 39, 34; 31 and QAX-0260, 0233, 0213, 0192, 0144, 0108, 0093; 0064. These letters documented that the ANI, a representative of Hartford Steam Boiler I&I Company, had approved QA Manual changes pertaining to Show Cause Order, Item V.A.(6)
and other Show Cause Order Items.
M33 This commitment stated, " Personnel training in DAG procedures will be conducted and procedures imple-mented in early September 1980".

The RRI reviewed training records which showed that training occ.urred on QAP-15.4, Data Analysis (Trending),

on August 22 and September 3, 1980 and on QAP-15.4, Rev. 1 on January 14 and 29, 1981.
M34 This commitment stated, "DAG first quarterly report
is expected to be issued for the fourth quarter 1980".
The RRI reviewed the initial quarterly report, dated November 21, 1980, which was issued for the third quarter of 1980. The next report, dated January 27, 1981, was issued for the fourth quarter of 1980.
Such trend a.:alyses reports have been and will continue to be furnished to HL&P and B&R management each quarter.

The RRI received objective evidence transmitted by and described by B&R Interoffice Memo STQ-8980, dated April 27,1981. This correspondence documented the training of Houston and site audit personnel relative to B&R Procedures ST-QAP-18.1 and 2.4.
A short.aurse in NDE was also conducted. Attendance lists showed that personnel had received this training.
Additional training in the codes is presently scheduled for May 4-8 and 26-28, 1981. Other training is planned

and will be scheduled in the near future to fully meet this commitment.
Status of Show Cause Order Imolementation On April 22, 1981, a meeting was held in the HL&P Senior Vice President's office to discuss the implementation of commitments H12, H13, A153, M4 and M42. The licensee clarifiec how these items will be implemented

and gave target dates for implementatitn. The licensee will document the status of all items in a letter to Region IV management.

Licensee Action on Previously Identified Items The RRI met with licensee personnel to review the status of noncompliances and unresolved items. The RRI was unable to complete the review of items 80-18-B, 80-18-C and 80-18-01 because the licensee was still in the process of resolving these items. These items will be reviewed during a future

inspection.
4.
Site Tour The RRI inspected storage laydown areas A, B, C, D, E, F, G, H, I and J.
The rebar laydown areas, tank ya/d and aggregate storage areas were also inspected.
No violations or deviations were identified during the inspections described in the above paragraphs.
5.
Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, violation, or deviations. One unresolved item disclosed during the inspection is discussed in paragraph 2.
6.
Manacement Meetinas The RRI met with one or more of the persons identified in paragraph 1 on April 21, 22, 28 and 29,1981 to discuss inspection findings and to discuss licensee actions and positions.
