Areas Inspected:

Routine inspection by the Resident Reactor Inspector (RRI) of safety-related construction activities including follow up on unresolved matters (concrete), RCB-2 liner plate welding, soil compaction and compaction test, safety-related structural concrete, plant tour and reactive inspection (allegations).

The inspection involved nineteen inspector-hours by one inspector.

Results:

No items of noncompliance, deviations or unresolved matters were identified.

DETAILS 1.

  • L. D. Wilson, Site QA Supervisor The Resident Reactor Inspector (RRI) also interviewed other licensee and contractor personnel during the subject inspection period.
  • Denotes personnel attending the weekly meeting with the RRI.

Licensee Action on Previous Findings (Open) Unresolved Item (50-499/79-15-2):

The RRI was advised that the subject unresolved matter concerning protection of concrete placement from weather was in review. The licensee representative stated that the concrete procedure was being revised to assure that the procedure included adequate plans to protect the concrete and/or test concrete which might be adversely affected by rain or other adverse weather.

This matter remains open pending review of the revised procedure.

Reactor Containment Building No. 2 (RCB-2) Dome Liner Plate Welding The RRI observed welding per B&R Procedure No. WPS-13 on Sections S-3-5, S-3-6, S-3-7, and S-3-8 as described in Roof Plate Layout Drawings No. E77AB, Course 3, E78B, Course 2, E79AB, Course 3, and other applicable course drawings.

The drawings specified weld joint design, marking, field stiffening, weld symbol and weld rod.

Each section contained two longitudinal welds to make a section.

The following specifics were inspected:

4.

Essential Cooling Water (ECW) Line Handline and Installation a.

The RRI observed the handling and storage of ECW 30" Aluminum Bronze thin wall pipe.

The RRI observed pipe that had been mishandled near the ECP intake structure.

NCR No. 3240 had been written and the damaged pipe was in the process of being removed. Another truckload was about to be unloaded but the off loading ceased until additional equipment arrived.

The RRI observed a repair weld in progress west of RCB-1.

The ECW line weld repair was No. 0007.

Records indicated preper inspection and NDE testing. Another veld was in process at the intake structure.

The RRI observed that the wind was becoming high and there was no shielding.

The welder stopped and volun-tarily placed a " wind box" over the area to control the wind.

No items of noncompliance or deviations were identified.

5.

Soil compaction The RRI observed soil compaction activities around RCB-2 and the floor of ECW intake structure.

The test sample for compartion was observed being taken at the ECW structure.

No items of noncompliance or devi?tions were identified.

6.

Safety-Related Structural Concrete The RRI observed concrete placed in Mechanical, Electrical, Auxiliary Building-Unit 1, (MEAB-1) wall.

Truck No. 68 had a sample removed and was tested.

The delivery ticket was 39193, and the design mix was B-1-3-11 (8 cubic yard).

The slump, air and te=perature measurements were acceptable.

No items of noncompliance or deviations were identfied.

Plant Tour The RRI toured the several areas to randomly inspect the following areas for storage, handling and housekeeping:

a.

RCB-1 and RCB-2 b.

MEAB-1 and MEAB-2

Fuel Handling Buildings, Units 1 and 2.

8.

Management Meeting The RRI met periodically with licensee representatives during the subject inspection period and discussed the inspections findings.
