Dear Kent Scott:

On December 31, 2025, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at the Callaway Plant. On January 6, 2026, the NRC inspectors discussed the results of this inspection with J. Morton, Supervising Engineer, Regulatory Affairs, and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at the Callaway Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information.

No findings or violations of more than minor significance were identified.

PLANT STATUS

The Callaway Plant began the inspection period at rated thermal power. On November 12, 2025, the licensee entered a forced outage due to secondary chemistry exceeding allowable limits following an inadvertent actuation of the auxiliary feedwater system. The unit was taken to Mode 5 where it remained until November 25 when plant startup was commenced. The plant was brought critical on November 29, the main generator was synchronized to the grid on November 30, and full power operations resumed on December 2. The plant remained at rated thermal power for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

(1) The inspectors reviewed the licensees corrective action program to identify potential trends in human performance that might be indicative of a more significant safety issue with emphasis on human performance errors during maintenance activities. The inspectors sampled a population of condition reports during the period of July 1 to December 31, 2025. Based on the samples reviewed, the inspectors identified seven instances where mispositioning occurred during maintenance activities and one example of late technical specification entry. While degradation was apparent in the implementation of the licensees human performance program, the licensees organization successfully identified these degradations before any harmful events occurred. The licensee independently identified this human performance trend, condition report 202506087 and condition report 202504781, and is taking appropriate actions to address the degraded conditions that were encountered during the inspection period.
