This area was re-radiographed utilizing very fine grain film to obtain better resolution, sensitivity, and definition in order to resolve linear indications. The radiograph revealed that the linear indications were cracking and areas of non-fusion. This item has been resolved and a NCR

This item will remain open until weld repair has been completed and re-inspected.

Repair welding of Unit 2 girder welds is in process and is expected i
to be completed by June 11, 1982. A UT of the Unit 1 girder welds is scheduled to be started by April 27, 1982. This UT is to be conducted on a random basis to determine if Unit I has similar defects as identi-fled on Unit 2 polar crane girder.
If similar defects are identified,
then a separate NCR will be written to identify these defects.

During repair operations of Unit 2 polar crane girder, it was identi-fled that a number of subsurface linear defects existed in shop welds.
The amount of repair work raised concerns about the overall quality of the girder assemblies. These concerns resulted in a meeting between CEI and NNI in which NNI agreed to study the problem and provide a recommended course of action for resolution. The owner concluded that a problem did exist and recommended that an informational UT be per-formed on all major structural welds in question. Joints were 100%
UT inspected with the exception of excavated joints undergoing repair
prior to the suspension of work activities. Of the approximately 100 UT reports resulting from this inspection effort, all indicated a reject disposition.

Containment Radiographic Review The above three personnel have been retained by Project Organization NDE Group to review 100% of the radiographs of Units 1 and 2 contain-ments.
An NCR has been initiated (T/AS0001). The review of the radiographs to date indicate that 62% of the welds reviewed are re-jectable. These rejectable welds pertain to the welds on NCR's No.17-152, 17-777,17-195, 17-254 and 17-268.
The review of the welds not concerned with the original NCR's indicates a reject rate
of 32%.

It is estimated that it will take a minimum of 90 days to completely review all the radiographs pertaining to Unit 1 and 2 containments.
Exit Interview The inspector met with site representatives (denoted in Perscns Contacted paragraph) at the conclusion of the inspection. The inspector summarized the scope and findings of the inspection noted in this report.

E. H. Nightingale of this office on April 21-23, 1982, of activitier. at the Perry Nuclear Power Plant, Units 1 and 2, authorized by NRC Construc-tion Permits No. CPPR-148 and No. CPPR-149 and to the discussion of our findings with Mr. R. Farrell and others of your staff at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during the inspection. Within these areas, the inspection consisted of a selective examination of procedures and representative records, observations, and interviews with personnel.
No items of noncompliance with NRC requirements were identified during the course of this inspection.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
If this report contains any information that you (or your contractors) believe to be exempt from disclosure under 10 CFR 9.5(a)(4), it is necessary that you (a) notify this office by tele-phone within ten (10) days from the date of this letter of your intention to file a request for withholding; and (b) submit within twenty-five (25)
