. Cordell Williams, Reactor Construction and Engineering Support Projects Section No. 2 Investigation Summary: Investigation on September 25 - 26, 1979 (Report No. 50-440/79-08 and 50-441/79-08).

Areas Investigated: An anonymous individual alleged two techinicians employed by U.S. Testing Company at the Perry site had performed work for which they were not qualified. The investigation of this allegation consisted of a review of pertinent procedures and records and interviews of personnel and involved eight investigation hours by one investigator.

Results: The allegation was not substantiated and no items of noncom-pliance were identified.

. REASON FOR INVESTIGATION . An unsigned letter dated June 23, 1979 was received by Region III al-legingthattechniciansemployedbyU.S.TestingCompanyattheferry Nuclear Power Plant site performed tests before documentation r(flected gthey were trained and qualified to perform them.

-I } t SUMMARY OF FACTS An anonymous letter dated June 23, 1979 was received by Region III al-leging U.S. Testing Company personnel at the Perry site were performing tests before documentation reflected they were qualified to perform them.

Copies of several U.S. Testing Company records relating to two specific individuals were enclosed with the letter.

The investigation determined that one individual was employed for a few days. During this time he performed a test in the field under the sur-veillance of the laboratory supervisor and was fired for inadequate perfo rmance.

The file of the second individual will show he was a qualified Level 2 Inspector at the time he performed tests corresponding to the copies of records furnished by the alleger.

Based upon an examination of these two specific examples, the allegation was not substantiated.

An overall evaluation of the training and qual-ification program of U.S. Testing was not undertaken during this in-vestigation.

No items of noncompliance were identified.

Introduction By letter dated June 23, 1979 an anonymous individual provided copies of records relating to the qualifications of two technicians employed by the U.S. Testing Company (UST) at the Perry Nuclear Power Plant (PNPP) as well as records of tests on which their names appeared. These records were supplied to demonstrate that the in-dividuals performed functions before they were qualified for them.

Copies of qualification records and test documents supplied by the anonymous alleger relating to individual A are attached to this report as Exhibit A.

Copies of similar documents supplied by the alleger relating to Individual B are attached as Exhibit B.

This investigation was limited to an examination of the qualifica-tions and work of those two individuals.

Qualifications and Work Performed By Individual A The UST Lab Supervisor advised that Individual A had previously been employed by UST at the Midland site. After an extended leave of absence he was re-hired by UST for work at PNPP. After Individual A had undergone a few days of orientation and appeared to understand his job, the Lab Supervisor prepared a Proficiency Evaluation Record dated April 9, 1979.

Individual A then worked with other inspectors l performing a few tests. The inspectors who worked with Individual A Y informed the Lab Supervisor that they were of the opinion that - { Individual A did not know how to perform the work. The Lab; Supervisor i-said he assigned Individual A to perform a test in the field and then observed his actions from a distance. On the basis ofihis observations, the Lab Supervisor concluded that Individual b's performance was unsatisfactory and fired him.

The Lab Supervisor stated he subsequently learned that Individual A had suffered a nervous breakdown following his employment at Midland.

He said he had concluded that Individual A's inability to perform as an inspector at PNPP after having performed the same work at Midland was due to his illness.

3 t It was noted that UST files contained a Documentation of Qpalifications - I certifying Individual A as a Level IB inspector which was dated t April 12, 1979. A copy of this record was obtained and is attached to this report as Exhibit C.

The Lab Supervisor had no explanation as to why this document was dated after Individual A had performed some inspection work on April 9 and 11, 1979 and his performance was found to be unsatis-factory. The Lab Supervisor however, pointed out that Individual A had only worked two or three days with other inspectors before he was terminated on April 13, 1979.

Qualifications of Work Performed By Individual B UST files regarding Individual B contained records showing he was certified as a Level I inspector on August 29, 1977 and as a Level II on April 24, 1978. The Level II Documentation of Qualifications was updated on November 13, 1978 and again on March 26, 1979.

Copies of these documents were obtained and are attached to this report as Exhibit D.

It was noted that the earlier Level II Documentation of Qualification records, those dated April 24, 1978 and November 13, 1978 had not been among the materials supplied by the anonymous alleger. These Documentations of Qualification predate the January,1979 test records on which Individual B's name appears as a Level II inspector.

It was noted however, that none of Individual B's Documentations of Qualification indicate he was certified for test ASTM C-127-68 or C-127-73.

On the January, 1979 record for these tests (Exhibit B, page 4) which was supplied by the alleger Individual B'r; nam appears in the " checked by" blank. The Lab Supervisor stated that.he test was performed, as indicated on the test record, by a qualified Level II inspector and that Individual B had merely checked the mathematical calculations of the data entered on the record. He stated that an individual performing this function need not be qualified to perform the test.

It was ascertained that Individual B is no longer employed by UST having terminated his employment on or about May 25, 1979.

Close-Out Discussion I i ( On September 26, 1979 the investigation findings were discussed with the following site personnel: R. L. Vondrasek, J. J. Connelly, and M. Kritzer.

It was indicated that although some discrepancies were noted in the records relating to Individual A, his employmeIlt at the-4-1608 215

~ site was brief and there was not sufficient basis for a finding of noncompliance.

It was further indicated that although some ad-ditional records and related procedures had been reviewed, the in-vestigation had been limited to a review of the qualificati,ons and work performed by two former UST employees only.
