The simulator portion of the operating examination was administered to one reactor operator (RO) who had previously failed an NRC administered requalification examination.
The examination was graded concurrently by the NRC and the facility training staff.
As graded by the facility and the NRC, this RO passed the simulator portion of the examination.
Although not speci-fically evaluated by the NRC, the crew was rated as, performing satisfactorily during the simulator portion of the examination.
The licensed operator requali-fication retraining was found to be effective.

REPORT DETAILS 1.
INTRODUCTION During the examination period, NRC examiner s conducted a requalification retake examination of the simulator portion of the operating examination for one Reactor Operator (RO) at the Susquehanna Steam Electric Station.
The examiners used the process and criteria of NUREG-1021,
"Operator Licensing Examiner Standards (Revision 5, January 1, 1989)" ES-601,
"Administration of NRC Requalification Program Evaluation."
The following licensee persons contacted during the examination provided key information. All persons attended the exit meeting (paragraph 4).

GENERAL OBSERVATIONS A.
Individual weaknesses previously noted for the RO given the retake examination were effectively corrected by the facility's training program.
No additional weaknesses were noted.
B.
Overall crew performance was assessed as satisfactory.
In general, recognition of procedure entry symptoms and procedure use was good.
Crew teamwork was also good.
There were some examples where the crew failed to fully assess conditions in order to diagnose problems, but there were other cases where assessments were adequately performe.
EXIT MEETING On August 8, 1989, the chief examiner conducted an exit interview at the conclusion of the examination.
The operations and training staffs were thanked for their participation in the examination.
Observations noted during the examination were discussed.
