License NPF-1 Attachment A Page 1 of 2 EEPLY TO NOTICE OF VIOLATION Violation 93-06-01 Title 10, Code of Federal Regulations, Part 50 (10 CFR 50), Appendix B, Criterion V states in part:

" Activities af fecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances, and shall be accomplished in accordanca with these instructions, procedures, and drawings."

Trojan Plant Procedure (TPP) 13-5, Revision 0, dated May 29, 1991,

" Control of In-Plant Unsecured Equipment / Material", requires that all unattended equipment / material which has sufficient mass and mobility to damage safety-related equipment during a seismic event and is located in an area within the scope of the procedure, includi_g the area surrounding the Spent fuel Pool, shall be properly secured. Step 4.2.8 states,

" Portable laddera shall be tied down or laid down when left unattended... "

Contrary to the above:

On Apri*. ?7. 1993, three portable ladders had been left unattended in the area rurrovtding the Spent Fuel Pool.

Reason for the Violation The reason for the violation was personnel error, failure to follow the procedure as written. Trojan Plant Procedure (TPP) 13-5 required ladders to be tied down or laid down when left unattended.

Corrective Steps That Have Peen Taken and the Riaults Achieved

The scaffold ladders were laid on the floor to comply with the a.

At a later date, the scaffolding and scaffold ladders were removed to a proper storage location outside the area of the Spent Fuel Pool.
