Trojan Nuclear Plant Document Control Desk Docket 50-344 April 27, 1988 License NPF-1 Attachment Page 1 of 2 REPLY TO A NOTICE OF VIOLATION Violation (Trojan) Technical Specification 6.11 requires that procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shat.1 be approved, maintained, and adhered to for all operations involving personnel radiation exposure.

The licensee's Radiation Protection Manual, established pursuant to the above Technical Specification,Section II.D.2.e states, in part, that hot spots will be posted when the contact radiation level is equal to or greater than five times the general area radiation level and the contact radiation level exceeds 100 mrem /hr.

Contrary to the above, on February 9,1988, in a work area, with a general background radiation level of 15 mrem /hr, four hot spots that exhibited contact radiation levels from 800 mrem /hr to 1200 mrem /he were not posted.

This is a Severity Level V Violation (Supplement IV).

Response Portland General Electric Company (PCE) acknowledges the violation.

1. Reason for Violation The reason for the violation was personnel error in that the Radiation Protection technician assigned to the radwaste system modification work being performed in the area of the hot spots failed to survey and label the hot spots, as required by the Radiation Protection Manual.

2. Corrective Action Taken and Results Achieved All work in progress in the vicinity of the hot spots was immediately halted. Access to the area was restricted by locking the door (to the pipe chase area), with the key controlled by the Operations Shift Super-visor. The area was resurveyed by Radiation Protection personnel and the hot spots were labelled in accordance with approved radiation protection procedures. Other accessible areas of the Plant with potential hot spots have subsequently been resurveyed and hot spot labelling has been corrected as required.

d 3. Corrective Action Taken to Avoid Further Violations a. The contract Radiation Protection technician involved in the violation was counselled on the procedural requirements for properly locating and labelling radiation hot spots, and on requirements for following procedures. He was also given a disciplinary letter.

Trojan procedural requirements for locating and labelling radiation hot spots were discussed at a staff meeting with the Radiation Protection technicians.

c. The Radiation Protection Manual and implementing procedures will be revised to more clearly define the requirements for locating and labelling radiation hot spots.

4. Date When Full Compliance Will Be Achieved Corrective action for Item 3.c will be completed by June 30, 1988. The other corrective actions have been implemented, and full compliance has been achieved.
