Discussion A brief summary of the Systematic Assessment of Licensee Performance

(SALP) Program was presented to explain the basis and purpose of the evaluation.
The NRC Region I evaluation board meeting was discussed, including the assessment period, performance data and the results of the board's evaluation.
The licensee's overall performance is rated average.
Twenty-five functional areas as well as facility management were evaluated.
The Itcensee's performance was rated below average in two functional areas.
Nine functional areas were not rated because the activity is essentially complete, the activity has not started, or activity is at too low a level to provide a basis for evaluation.

(COR's) were submitted by the licensee; all of them the result of design /
manufacturing errors.

Causally Linked Events There were four CORs. All involved manufacturing defects.
Two involved Square-O motor control centers, with the problems being improper lubrication.of rubber bumpers on movable starter armatures

(design control inadequacy) and improper terminations (manufacturer QC inadequacy). The other two CORs were based on manufacturer modifications. One involved Bailey concrollers, with power supply capacitor damage caused by contamination from packing materials.
The other was a generic design defect (inadequate thrust bearing lubrication) in the DeLaval Diesel Generators.
The following repetitive factors were identifed:

The small number of CORs prevents definitive conclusion.
Routine vendor checks should be capable of determining if the Square-D personnel error (terminations) and management error (design control)

Licensee Activities

During the assessment period the licensee's efforts have been concentrated primarily in two areas. The first area is an effort to complete final construction of the various' systems and subsystems and turn the systems over from construction to startup for preoperational testing.
The bulk of the-remaining work is in the area of electrical, instrumentation, and pipe hangers, supports and restraint. At the end of February, 1981 189 of a total 287 systems / subsystems had been turned over to startup. The second major effort was to perform the Integrated Systems Flush, which would involve the Reactor Vessel and attached systems and which is the first major milestone identified on the licensee's critical path schedule to fuel load. Current projects call for its performance in April and May, 1981 about 2 months behind schedule.
Licensee management has esti-mated fuel load would occur between June and September, 1982. The NRC Case Load Panel reviewed construction status in May,1980 and estimated a fuel load date of September, 1982.
Site work force averaged approximately 3200 people during the assessment pariod; 2300 manual, 700 non-nanual, and 200 QA/QC, As of December 1980 the licensee estimated that the plant was 88% complete.

Inspection Activities Construction Phase During the assessment period there were seven region based construction inspections comprising 264 inspection hours.
Functional-areas inspected were primarily instrumentation, electrical, pipe hangers and supports, and preservice inspection.
Project inspection areas'have been electrical, pipe hangers and supports, review of construc-tion deficiency reports, IE Bulletins and Circulars and followup on previous inspection findings t.id allegations. With the exception of the electrical, instrumentation, and pipe hangers, supports, and restraints, the construction inspection program is essentially complete.
Preoperational phase

There has been one preoperational phase resident inspector at the site

since September 30, 1979. During the assessment period the resident
,
inspector has performed inspections in the construction, preoperational and permanent plant staff areas.
Resident inspection hours totaled 759 for the period. Additionally during the period there was one inspection by a region based preoperational test inspector and one by the environmental section.
Note: Overall total inspection hours during the asessment period were 1144.
