SUBJECT: NRC INSPECTION REPORT NOS. 50-321/93-07 AND 50-366/93-07 Thank you for your response of June 28, 1993, to our Notice of Violation,

issued on June 4, 1993, concerning activities conducted at your Hatch facility. We have evaluated your response and are concerned that the

.

potential causal factors for the violation may not have been fully addressed.

The violation, regarding an unauthorized entry into a posted high radiation area, appears to indicate a lack of sufficient attention toward licensed

>

responsibilities by plant workers who have been trained to understaad these requirements.

Although this violation did not lead to a radiation exposure l

which exceeded regulatory limits, violations of this type create the potential i

for significant exposures to personnel making unauthorized entries into high

radiation areas.

We are concerned that, as with previous violations regarding inappropriate

!

entries into posted high radiation areas, the corrective actions taken in

,

response to this violation, which consisted of disciplinary actions and

issuance of a memorandum from plant management regarding the importance of l

compliance with plant postings, may not be sufficient to prevent future noncompliances with licensed requirements for high radiation area entries.

During a July 15, 1993 teleconference between Wayne Kirkley and Dorsey Smith of your staff, and Elizabeth Pharr and William Cline, NRC RII, we discussed our concerns as related to your response to this Notice of Violation. Our concerns were acknowledged and we were informed of continuing efforts being

taken by your staff to improve worker compliance with radiological barriers.

Further, we would encourage you to continue to emphasize the importance of adherence to established health physics controls' and radiological barriers through your general employee and technical training programs.
