This refers to our letter dated March 8, 1988, which forwarded the results of a special safety inspection conducted to examine your fuse-breaker coordination studies as they related to the 250 VDC safety busses and a review of a Licensee Event Report (LER) associated with the Local Shutdown and Indication (LSI) panels.

The NRC Inspection Reports 50-315/88003(DRS)

and 50-316/88004(DRS)

indicated there was one apparent violation regarding the failure to implement adequate design control measures.

The letter also stated that we were evaluating appropriate enforcement actions for that violation.

The evaluation has been completed and the NRC staff has determined that certain of your activities appear to be in violation of NRC requirements, as specified in the enclosed Notice.

This violation consists of three examples where inadequate design control measures were applied.

The first example was attributed to an oversight by design engineers in the design and engineering process associated with the initial Appendix R modifications to the LSI panels; the second example was the application of a fuse-breaker selectivity ratio that was not verified to be adequate prior to implementing the design change to correct the design deficiency discovered in the LSI panels; and the third example was the inadequate fuse-breaker coordination between the Essential Safety System (ESS)

loads and Balance of Plant (BOP) loads on the 250 VDC safety-related busses that had existed since each unit's initial startup.

We have considered the additional information you provided at an Enforcement Conference held in the Region III office on March 17, 1988, between you and members of your staff and Mr. C. J. Paperiello and members of the Region III staff.

Your presentation indicated that the occurrence of any of the above examples concurrent with any analyzed operational event requiring the above equipment to mitigate that event was highly unlikely.

Cook units had experienced a loss of electrical power to the above equipment as a result of an electrical fault.

We therefore categorize this as a Severity Level IV violation.

The NRC is concerned with how effectively design control measures are being applied throughout your engineering organization.

In addition, we are concerned with the process of selecting qualified individuals that provide design input and perform the design review.

Therefore, in addition to describing your specific action to correct the noted violation, please provide us with the steps you will take to preclude further instances of inadequate design control in the other engineering disciplines.

Further, during the Enforcement Conference, you committed to submit to us your Safety System Functional Inspection (SSFI) evaluation and provide us a copy of your plans to address generic design control issues identified by the SSFI, including confirming that no other electrical coordination discrepancies exist.

Your reply to this letter and the results of future inspections in this area will be considered in determining whether escalated enforcement actions may be appropriate if similar violations are identified during future inspections.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, the enclosure, and your response to this letter will be placed in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this matter.
