The Directorate of Regulatory Operatione Region II, was notified by telecon from M. H. Kleinman, Manager, Quuity Programs and Standards, Florida Pover Corporation, on August 11, 1972, that a void in the reactor building' concrete structure had been discovered following form removal.

The extent of the void was not known at the time of the telecon. The void was observed during a routine unannounced inspection at the site on August 15-17, 1972.

When the forms were removed on August 9,1972, from concrete which had been placed in" the ' reactor building cylindrical concrete structure, in the vicinity of the personnel access tube, a void was found in the concrete.

The void is located immediately ander the access tube. Dimensions of the void at the structure's outer surface are approximately 18 inches in length by 6. inches in~ height. Subsequent examination revealed that the void ex-tended completely through the reactor building vall to the inner liner plate. An exploratory opening has been cut in the reactor building liner

. plate to determine the ext' 't of the void. The void shape very roughly resembles a conical section. Dimensions of the void at the reactor building liner plate surface are approximately four feet in height by two feet in vidth.. The personnel access tube forms the upper boundary of the void.

The architect-engineer (Gilbert Associates, Inc.) is conducting an investigation to determine the extent of the problem. When the extent of the prGblem is known and a repair procedure is decided upon, Florida Power Corporation has agreed to notify the Directorate of Regulatory Operations, Region II, prior to uking the actual repair.
