Thank you for your letter' dated March 7, 1989, informing us of the steps you-'
.~have taken to correct the violations identified in Inspection Report No.
50-295/88021(DRF); 50-304/88021(DRP) forwarded by our letter dated February 7, 1989. Your response: acknowledged.two violations, 1) that quality assurance (QA) program procedures were not followed in that neither a DVR nor a DR was initiated when all 20 main. steam safety valves (MSSVs) were tested and found
.to be out of tolerance for lift setpoints, and 2) that purchase documents for.-
the MSSV testing failed to be properly marked as _ASME Section XI.for code related_ testing or to include all technical engineering requirements.

importance of proper documentation of events, including QA's responsibility to assure that DRs or DVRs were initiated, and processed promptly. We believe that similar lessons learned training should be applied to other individuals who are responsible for initiating DRs or DVRs.
In a conversation between Mr. M. Holzmer, the NRC senior resident inspector and Mr. W. Stone of your staff, on March 22.and 23, 1989 you agreed to write a letter to all department heads at'the Zion Station to accomplish this purpose.
In response to the second violation you committed to instruct QC personnel to
. question the purchase order originator when QC is unsure whether all require-ments are listed or referenced. We believe that all reviewers of purchase orders should be similarly trained.
In a conversation between Mr. M. Holzmer, the NRC senior resident inspector and Mr. W. Stone of your staff, on March 22 and 23, 1989 you stated that this had been covered in one of your department head meetings and you agreed to provide documentation that all department heads have received this training.
In the event that you cannot find the necessary minutes of the meeting, you agreed to hold retre:ning with the department heads and to document the same.
You are requested to submit to this office, within 45 days of the date of this letter, a written statement of verification of the commitments stated abcve.
