Organization and Management Controls The NRC inspectors reviewed the FCS staff functional assignments regarding the radioactive waste program. The NRC inspectors determined that FCS had identified in Standing Order T-2, " Waste Liquid Release," Standing Order T-3, " Waste Gas Release," Standing Order T-4, " Waste Solids Release," and

" Radiation Protection Manual," Section 6.0, " Radioactive Material Control," that the supervisor, chemistry and radiation protection, and the plant health physicist were responsible for collection, compaction and packaging, and for assuring that offsite shipments of radioactive waste are conducted properly. The chemistry and radiation protection group and shift supervisors were responsible for the liquid and gaseous waste release The NRC inspectors reviewed the licensee's radioactive effluent release program and radioactive waste program and determined that written

-4-procedures were established for these program task The NRC inspectors noted that a radwaste coordinator position had been established in the C&RP organization and that an additional qualified ANSI N18.1-1971 C&RP technician had been assigned to the radwaste organizatio The NRC inspectors noted that no turnover of personnel had occurred in the radwaste organization since the previous inspection of this area. The licensee's policies, administrative procedures, position descriptions, and current functional area assignments for the radwaste organization appeared to be sufficient for assuring effective control of radwaste activitie No violations or deviations were identifie . Training ard Qualifications The NRC inspectors reviewed the training activities related to the radwaste program to determine conformance with Section 5 of the Technical Specifications (TS).

The NRC inspectors noted that the entire radwaste organization had attended a radwaste-transportation seminar presented by a contractor. The radwaste coordinator had also attended an offsite radwaste management meeting in 1985. A member of the radwaste organization had also attended a one week offsite radioactive waste management seminar. The NRC inspectors reviewed the licensee's training for compaction of radioactive waste, waste drum preparation, and decontamination wor During the exit interview, the NRC inspectors expressed a concern to the licensee about the lack of coordination between the FCS training organization and the radwaste group concerning training activities. The NRC inspector noted that the training organization was not always informed of training activities initiated by the radwaste organizatio No violations or deviations were identifie . Radioactive Effluent Releases The NRC inspectors reviewed the licensee's liquid effluent release program to determine compliance with TS 2.9 and 3.12 and Table 3-11. The NRC inspectors reviewed the licensee's records for radioactive liquid releases which are listed in the Attachment, Section The NRC inspectors also reviewed the licensee's gaseous effluent release program to determine compliance with TS 2.9 and 3.12 and TS Table 3-12. The NRC inspectors reviewed selected licensee's records for radioactive gas releases. -5-5. Records and Reports of Radioactive Effluents The NRC inspectors reviewed the licensee's records and reports to determine compliance with the requirements of 10 CFR 50.36(a)(1) and (2)

and TS 5.9.3.f, 5.9.4, and 5.10. The NRC inspectors reviewed the semiannual reports for: July 1 - December 31, 1983; January 1 - June 30,1984; and July 1 - December 31, 1984. The annual reports for 1983 and 1984 were also reviewe These reports were in the format of NRC Regulatory Guide 1.2 The NRC inspectors reviewed the FCS licensee event reports (LERs) that had been issued since the previous inspection of this area that involved radwaste activities. The following is a listing of the LERs:

  • 84-004 Reactor coolant dose equivalent iodine limit was exceede * 84-011 Auxiliary ventilation discharge duct was disconnected from RM-061 and RM-062 during containment release of pressur * 84-016 Surveillance Test RM-2 determined set point for RM-062 exceeded high alarm set poin * 84-020 Personnel did not follow procedure in servicing the hydrogen analyze * 84-025 Gas leaks in waste gas vent header caused stack iodine monitor (RM-060) to initiate the ventilation isolation actuation signal (VIAS).

Effluent Control Instrumentation The NRC inspectors reviewed the licensee's effluent monitoring instrumentation and procedures used in calibration of effluent monitors and for controlling effluent releases to determine compliance with Updated Safety Analysis Report (USAR) commitments and TS requirement The NRC inspectors reviewed selected procedures related to the effluent control instrumentation which are listed in the Attachment, Section . -- - _

-6-The NRC inspectors noted that the licensee had used new revisions to the calibration procedures which incorporated electronic, secondary, and primary calibrations using radioactive standards in the same media (liquid and gases) as the actual samples being measured in the effluent streams. The radioactive standards used were traceable to the National Bureau of Standards and were of various activity levels to cover the full range of each monito No violations or deviations were identifie . Effluent Release Procedures The NRC inspectors reviewed the licensee's effluent release procedures to determine compliance with TS 2.9, 3.12, and 5.8. Selected documents listed the Attachment, Section 4 were reviewed by the NRC inspector The NRC inspectors reviewed the procedures governing the release of radioactive effluents. These effluent release procedures provide for the following: sampling of effluent and radionuclide analysis prior to release; testing effluent isolation valves and effluent monitors prior to release; recording dilution and meteorological parameters during releases; verifying discharge flow rates, chemical concentrations, and dilution; control of waste gas discharges for gas decay tanks held for less than 30 days; and the use of checklists for the operation of both liquid and gas waste effluent discharge systems including shift supervisor's signatur No violations or deviations were identifie . Testing of Air Cleaning Systems The NRC inspectors reviewed the licensee's procedures and selected records for maintenance and testing of control room, spent fuel facility, safety injection pump room, and containment air cleaning systems to determine compliance with TS 3.2 and 3.6(5).

The licensee's procedures provided for the required periodic functional checking of system components, evaluation of charcoal adsorber iodine removal efficiency, tracking of system operating hours, and annual refueling outage replacement and inplace testing of certain filter systems. A review of past records showed that inspections and tests required by the TS had been completed in a timely manner. A selection of the records and documents listed in the Attachment, Section 5, were reviewed by the NRC inspector No violations or deviations were identifie . Reactor Coolant and Secondary Water Quality The NRC inspectors reviewed the licensee's program for monitoring and controlling the reactor coolant and secondary water quality to determine compliance with USAR commitments and TS requirement __

-7-The NRC inspectors reviewed the procedures and selected reactor coolant and secondary chemistry analyses records for 1984 and January-April 198 The procedures and records reviewed are listed in the Attachment, Section 6. The records indicated that all required sampling and analyses were performed at the frequencies required by the T No violations or deviations were identifie . Calibration and Quality Control Program of Analytical Measurements The NRC inspectors reviewed the licensee's program for calibration and quality control of chemistry and radiological analytical measurements to determine compliance with USAR commitments and TS requirement The NRC inspectors found the type and quantity of laboratory analytical instrumentation adequate to perform the radwaste analysis requirements specified in the T The NRC inspectors examined the licensee's laboratory and radiochemistry counting room instruments calibration and quality control procedures, calibration and function check data, and other documentation of instrument performance. Data for the year 1984 and the period January-April 1985 were reviewed. The licensee's records appeared to be in order, and data were presented according to procedure. The procedures and records reviewed are listed in the Attachment, Section No violations or deviations were identifie . QA Program The NRC inspectors reviewed the licensee's QA audit and surveillance program regarding radwaste and offluent release operations to determine compliance with USAR commitments, TS requirements, and QA Manua The NRC inspectors reviewed selected surveillances and audits conducted on radwaste control and waste management and control of effluent releases during the period from January 1984 through April 1985. The QA surveil-lances were performed according to their respective monthly, quarterly, or semiannual frequency. The QA surveillance program had not identified any significant problems. The NRC inspectors suggested that the licensee expand the containment purge surveillance checklist to be consistent with the waste gas decay tank release surveillance checklist to ensure that all TS requirements are me The licensee agreed to evaluate the surveillance checklists and to modify them if necessar The NRC inspectors reviewed the corporate QA audit schedule for 1983-1986 and verified that the corporate QA audits were being performed on a tri-annual cycle. Audits No. 56 and 57, " Radioactive Material Packaging, Shipping, Control and Waste Management," were conducted in August 198 The audit team included a technical specialist trained and knowledgeable in radwaste activities and transportation of radioactive materials at nuclear power facilities. The NRC inspector noted that audit findings had been corrected in a timely manne _

-8-Audit No. 55, " Environmental (Radioactive) Monitoring," conducted in September 1984, was also reviewed. The NRC inspectors noted that the audit checklist did not include a comprehensive listing of the areas being audited. The lack of a comprehensive checklist was discussed with a licensee representative who agreed to evaluate the scope and depth of the audit checklists and take appropriate action if necessar The procedures, surveillances, and audits reviewed are listed in the Attachment, Section No violations or deviations were identifie . Solid Radioactive Waste The licensee's program for solid waste disposal was last inspected and reported in NRC Inspection Report 50-285/84-24. The NRC inspectors reviewed selected records for radwaste shipments that had occurred since the above mentioned inspectio No violations or deviations were identifie . Independent Confirmatory Surveys The NRC inspectors made confirmatory radiation surveys to verify the licensee's survey result The NRC inspectors inspected the auxiliary building, radwaste building, spent fuel pool storage area, and restricted area outside of the plan The NRC inspectors made independent measurements in these areas, and determined that the designated areas in the radiation control area were properly posted and controlle During the inspection of the auxiliary building, the NRC inspectors noted that only one portable continuous air monitor was in operation. The NRC inspectors expressed their concern concerning the lack of a comprehensive continuous air monitoring program for the auxiliary building. A licensee representative stated that the NRC inspectors' concern would be reviewed before the next refueling outag No violations or deviations were identifie .
