Results:

Inservice Ins ection-Review of Pro ram (73051)
The inspector performed a general review of the basis of the licensee's ISI program and identified the following:
PG8E Letter No.
DCL-89-171, dated June 23, 1989, submitted Revision 4 of the Inservice Inspection and Testing Program Plans for Diablo "
Canyon Unit 2.
Revision 4 of the ISI Program Plan superseded the
'revious revisions in its entirety, with the exception of certain ISI boundary drawing pages in section 2.0, and included several requests for relief.
The Unit 2 ISI program is based on the requirements of the ASME Code,Section XI, 1977 Edition through and including the summer 1978 addenda.
The initial inspection interval began March 13, 1986 when Unit 2 was placed in commercial operation.
The Office of Nuclear Reactor Regulation reviewed Revision 4 of the Unit 2 ISI program plan, as supplemented by PG8E letters dated September 7 and 19, 1989, and issued a safety evaluation report on October 25, 1989.
No violations or deviations were identified in the areas reviewed.

Inservice Ins ection - Review of Procedures (73052 Inspection Report No.'50-275/89-28 identified that during the November 1989 review of Unit 1. ISI activities, the following was identified:
Some of the licensee ISI Ultrasonic (UT) procedures did not provide a place on the data sheets to record the actual surface temperature of the examination surfaces, for individual weld joints, piping, components, etc.
Multiple examination surfaces could be recorded on one data shee The licensee agreed to revise the applicable forms/data sheets as required to document the individual surface temperature of the examination surface, for each examination.
During this inspection it was identified that the licensee had revised the UT calibration and data sheets in the following procedures:
Procedure No. N-UT-1, Revision No. 5, "Ultrasonic Examination Procedure for Pipe Melds".
Procedure No. N-UT-4, Revision 1, "Ultrasonic Examination of Pressure Vessel Welds".
The licensee identified that the remaining UT procedures that did not get revised prior to the start of the Unit 2 outage, were going through review and would be issued soon.
No violations or deviations were identified in the areas reviewed.

Inservice Ins ection - Observation of Work and Work Activities (73753)
The licensee was conducting the Unit 2 third refueling outage.
This cycle 4 outage, was taking place during the second period of the first ten year ISI interval.
The inspector reviewed the qualifications and certification records for the ISI Examiners and the equipment certifications.
Available visual (VT), ultrasonic (UT) and liquid penetrant (PT) examinations performed on pressurizer piping and the spent fuel pool system piping were observed by the inspector.
No violations or deviations were identified in the areas reviewed.

Inservice Ins ection - Data Review and Evaluation (73755 The inspector reviewed available non-destructive examination (NDE) ISI calibration sheets, data sheets, and nonconformance reports generated this outage, and identified the following:
During the performance of UT examinations of pressurizer welds no.
and no.
10 on March 29, 1990, the latest effective UT examination data sheets (mentioned in paragraph 3 above)
were not used to record examination data:
Since the original form N-UT-48 data sheets were used instead of the new Attachment B,
Form N-UT-4B, Rev.
1 data sheet, the surface temperature of the examination surfaces were not recorded as required by revision no.
1 to procedure no.
N-UT-4, "Ultrasonic Examination of Pressure Vessel Melds".
Once the inspector identified the licensee had not recorded all the required U.T. examination data, since the old data sheet was incorrectly used, the licensee took the following actions:

Examined Control Room Logs for March 29, 1990 and identified that the applicable system temperatures for the pressurizer welds surfaces examined on that date were 88 degrees Fahrenheit.
Verified that the calibrations for temperature instrumentation (TR No.
636 and.No.
648)
used to measure the applicable March 29, 1990 temperatures, recorded in the control room logs, were acceptable.
The calibration due dates for this instrumentation were March 24, 1993.
Transferred the original March 29, 1990 U.T. examination data for pressurizer welds no.
1 and no.
10, to the correct revision 1, Attachment B, form N-UT-4B data sheets.
Entered the reconstituted surface temperatures for the examination surfaces on these new data sheets.
The failure to use the appropriate instructions (data sheets)
in accordance with 10 CFR Part 50, Appendix B, Criterion V, was an apparent violation (50-323/90-11-01).
Based on the corrective actions discussed above; that were taken by the licensee and verified during this inspection, no response is required.
This violation is not being cited, since the criteria set forth in 10 CFR Part 2, Appendix C, Section V.A.
were found to have been satisfied.
Exit Meetin (30703 The inspector met with the licensee management representatives denoted in paragraph 1,
on April 6, 1990.
The scope of the inspection and the inspector s findings up to the time of the meeting were discussed.
At this meeting the inspector identified that he had obtained some information that would be reviewed later in the reqion, with the findings documented in this report.
The available information was reviewed and the findings included in paragraphs 4 and 5 of this report.
