This refers to the telephone conversation on September 6, 1985,-betwea.. you and Dr. R. Bellamy of this office, regarding the deficiencies noted in your radiation protection program at Vermont Yankee Nuclear Power Station during the inspection conducted on August 9, 1985 (50-271/85-21).
The inspection reviewed a radiological incident in which a Chemistry & HP Tech-nician received an unpla'nned occupational radiation exposure of approximately 1.3 rem to the whole body while performing a radiation survey in the TIP room.
Although the radiation exposure received by the individual was not in excess of the regulatory limit, the causes for the exposure are of concern to the NRC because a significant radiation source term existed in the TIP room and pre-cautionary procedures were not adequately communicated to the Chemistry & HP Technician who entered the room. This created a substantial potential for radiation exposure in excess of regulatory limits.
With regard to the matter discussed, we understand that you have undertaken or will undertake and complete the following actions by October 4, 1985:
(1) HP staff and worker initial and refresher training program will be upgraded to emphasize precautionary actions and measures regarding entry into high radiation areas.
Instructions shall be included on the significance of high dose rates (greater than 10 R/hr). Records of the training including instruction lesson plans and the employee's demonstration of understanding of the precautionary actions and measures will be retained.
(2) Permanent administrative control procedures regarding TIP room entries will be issued. These procedures will be similar in content to the interim controls issued on August 9, 1985, and will specify dose rates that, if encountered, w'll cause activities to be terminated. The interim controls will remain in effect until the permanent controls are issued.
