This routine, announced inspection was conducted in the area of the licensed operator requalification (retraining) program during the period May 15-19, 199 The purpose of the inspection was to (1) verify that the licensee's requalification program for reactor operators (ROs) and senior reactor operators (SROs) ensures safe power plant operation by evaluating how well the individual operators and crews had mastered training objectives; and (2) to assess the effectiveness of the licensee's corrective actions taken in response to outstanding items opened in previous NRC inspection report Emphasis was placed in reviewing the corrective actions that were implemented by the licensee to ensure that the individuals who are licensed to operate the facility satisfy the conditions of their licenses as specified in 10 CFR 55.5 Enclosure PDR ADOCK 05000269

a. Summary The NRC inspectors conducted a routine, announced inspection of the Oconee Nuclear Station licensed operator requalification (retraining)
program during the period May 15-19, 1995. The purpose of the inspection was to (1) verify that the licensee's requalification program for reactor operators (ROs) and senior reactor operators (SROs) ensures safe power plant operation by evaluating how well the individual operators and crews had mastered training objectives; and (2) assess the effectiveness of the licensee's corrective actions taken in response to outstanding items from previous NRC inspections, particularly those that were implemented to ensure that the individuals who are licensed to operate the facility satisfy the conditions of their licenses as specified in 10 CFR 55.53. Based on a review of records and observation of examinations, the requalification examination activities were satisfactorily conducted. The inspectors also found the corrective actions taken by the licensee were satisfactor b. Examination Development The inspectors reviewed the licensee's requalification written and operating examinations for week one by comparing them to guidelines provided in the licensee's procedures and NUREG-1021, "Operator Licensing Examiner Standards," Revision 7. operators' ability to safely operate the plant. The simulator scenarios were challenging and at the appropriate difficulty leve The Job Performance Measure (JPM) sets contained an appropriate distribution of tasks with alternate path and time critical elements in each se The Sample plan was comprehensive and incorporated all of the guidelines contained in NUREG 1021 ES-601 Attachment 3. The selection of JPMs and active simulator examinations were governed by the Sample Plan as required by Procedure OTG-004, "Conduct of Periodic Training Licensed Operator Requalification."
The inspectors noted that the licensee had implemented a review process to ensure that the written, JPM, and simulator examinations had been screened for excessive test item overlap. Overall, the inspectors considered the sample plan's organization, degree of detail, clarity, and usefulness in constructing content-valid, reliable examinations a strength of the progra The inspectors also noted that the discrepancies concerning Procedure OTG-004, identified in NRC Inspection Report Nos. 50-269, 270, and 287/94-25, had been corrected. The inspectors concluded that the licensee's process for the development of requalification examinations was satisfactor c. Examination Administration The inspectors observed examination activities to assess the facility licensee's effectiveness in conducting written examinations and operating tests. The evaluators consistently administered the examinations using the established performance standards. The simulator evaluators determined areas in need of retraining through observation of operator performance. The JPM evaluators administered the JPMs in an unbiased, objective manner to assess the operators'
ability to perform the given task. The inspectors concluded that the licensee evaluators were effective in administering the requalification examinations to determine pass/fail decisions and in documenting weaknesses of the operator. Action on Previous Inspection Items (92901)
a. (Closed) VIO-50-269,270,287/94-25-01, "Failure to follow procedural instructions." This violation encompassed two examples of procedural noncompliance. Procedure OTG-004, Step 5.3.D.3.c.3, required that students place their initials and date in a roster in front of the Shift Review Schedule binder after reviewing the required materia The inspector found 21 instances where the licensed operators had not initialed the roster. In addition, one operator had participated in his annual exam without completing the review. The licensee identified the cause of this violation to be a lack of a formal program to verify completion of the Shift Review Schedule. Procedure OTG-004 was revised to require that the Operations Shift Manager and Enclosure

the exam proctor verify that the reviews are completed prior to an individual taking his weekly exams. In addition, this violation resulted from the failure to follow procedure ETQS 703, "Test Development," Step 5.3.4. The licensee had administered identical tests on Thursday and Friday of each classroom segment contrary to the requirements of this procedure. The procedure called for no more than a two-thirds duplication between the examinations. The licensee corrected the problem by administering only one examination at the end of each classroom week. Procedure OTG-004, Step 5.4.A.1, incorporated this change. The inspector verified that the procedural changes mentioned above were complete b. The licensee did not document that individuals had directly performed certain required control manipulations. Implementation procedures did not identify which control manipulations were required to be performed by all individuals and which were satisfied by crew participation. The inspector verified that the control manipulations had been grouped to indicate which ones required individual participation and which were crew oriented. The inspector also reviewed Procedure OTG-004 and noted that it included requirements for tracking the required control manipulations to individuals. Each operator has to document participation by initialing and dating an individual record shee c. (Closed) VIO-50-269,270,287/94-25-03, "Failure to notify the NRC within 30 days of a change in medical status of an operator and failure to identify medical limits outside of requirements."
This violation resulted from the licensee not notifying the NRC within the required 30 days that 16 operators needed to wear glasses in the performance of their duties. All 16 operators needed amendments to their licenses requiring corrective lenses be worn during the performance of their duties. It also dealt with the failure of the licensee's medical review process to identify that three operators did not meet the required medical standards. The inspector verified that Procedure OMP 1-12 defined departmental responsibilities which collectively defined an orderly process for detecting and reporting changes in medical status of operators. All of the delinquent reports have been processed to the NRC, and appropriate amendments to the operator's licenses have been issue d. This item addressed the failure of the licensee to ensure that the individuals that return to active status performed the necessary plant tours as required by 10 CFR 55.53(f). Operators had not been aware of this requirement and the reactivation procedure (OMP 1-12) did not contain adequate guidance on expectations and program requirement Enclosure

The inspector reviewed the revised procedure and found that it contained specific guidance on the requirements for conducting a plant tour. A checklist was included in the procedure which specified which areas of the plant must be toured prior to reactivating a licens e. (Closed) IFI 50-269,270,287/94-37-01, "Adequacy of Non-Licensed Operator (NLO) program lesson plans."
This item concerned the lack of detail in the NLO lesson plans to allow the instructors to present them consistently and effectively. Interviews with instructors indicated that the lesson plans were at a satisfactory level of detail to support NLO.instructio f. (Closed) VIO-50-269/94-300-02, "Failure to maintain control of Operations Management Procedures (OMPs)." The licensee did not control the OMPs, allowing a conflict between OMP 1-2 and OMP 2-1 on the authority required for overriding a safety function. The OMPs in question have been revised to contain this information in only one OMP. The OMP Periodic Review Checklist now contains a step to ensure that one OMP does not conflict with guidance in another OM. Exit Interview At the conclusion of the site visit, the inspectors met with representatives of the plant staff listed in paragraph one to discuss the results of the inspection. The licensee did not identify as proprietary any material provided to, or reviewed by the inspectors. The inspectors further discussed in detail the inspection findings listed below.
