ITE-112; (1) The design requires eight 3/4" bolts to

hold the plate, and only seven bolts were installed,

(2) the attachment plate warpped around the center line

of the snubber and created 1/8" to 1/4" gaps between the i
concrete ceiling and the plate surface all around.

Subsequent to the inspection, the licensee informed the inspector that seven bolts were sufficient for the design loading, and that the gaps will be grouted during next cold shutdown or refueling. This is an unresolved item.

for Unit 1; and dated Apr1125,1977 and April 19, 1978 for
Unit 2, and had the following comments:

The measurement of the large Anker-Holth snubbers was inconsistent and not in accordance with procedural instruction.
