Announced inspection to observe the ultrasonic examination of recirculation inlet nozzle safe-ends.
3.
Recirculation Inlet Nozzle Safe-End UT Examination In response to a Region II request, TVA agreed to ultrasonically examine five of ten recirculation inlet nozzle safe-ends in Unit l.
This examination was requested to ascertain whether any significant UT indications existed in the area of the attachment weld-made to attach the thermal sleeve to the internal surface of the safe-end.
Previous UT inspection was performed on five of the Unit 3 safe-ends resulting in no significant indications being detected.
This inspection was reported in Region II report No. 50-296/78-2 RII Report No. 50-259/78-35 I-2 The examinations were performed by the LMT Level III UT examiner, utilizing the same technique employed for Unit 3 inspection (i.e.,
utilizing dual pitch catch 1.5 MH transducers).
LMT procedure No.
UT-10, Revision 3 plus field change dated October 17, 1978 was utilized.
This procedure was developed in accordance with the ASME boiler and Pressure Vessel Code Section XI, 1974 edition plus addenda through the Summer of 1975.
Nozzles IN2A, C, E, G and J were inspected.
The NRC inspector witnessed inspection of nozzles IN2C, E, G and J.
All five nozzles showed a 360 degree low level ripple believed by the examiner to be caused by the thermal sleeve weld.
Slightly higher ripples were noticed on nozzles G and J.
An indication which peaked at 30 percent DAC at the evaluation level sensitivity was noted on J nozzle at the 9:00 position.
No signal travel was noted for this indication.
This'ndication was considered by the examiner, as a
preliminary evaluation, to be caused by weld geometry.
A spot indication was noted on G nozzle which peaked at 40 percent of DAC of evaluation level sensitivity at the 6:00 position.
No circumferen-tial length or travel was noted for this indication.
The examiner indicated that this indication appeared to be out of the weld area and was typical of indications found in nozzles and not normally considered to be significant.
No items of noncompliance or deviations were identified.
