NRC INSPECTION REPORT 50-244/98-02-REPLY
Dear Dr. Mecredy:
This letter refers to your May 6, 1998 correspondence, in response to our March 31, 1998 letter. This correspondence dealt with an event in which licensed reactor operators failed to notice and respond to an increase in reactor coolant pressure.
We have reviewed this matter in accordance with NRC inspection Manual Procedures 92901.
We concur with your assessment of the cause of the event.
In summary, those causes reflected the combined failure of the pressurizer defect switch and the failure of control room operators to monitor, detect and respond to related indications, parameter and alarms.
We consider the actions you have taken to address the root cause to be acceptable and willfurther review the effectiveness of these actions in a future inspection.
Thank you for informing us of the corrective and preventive actions documented in your letter. These actions will be examined during a future inspection of your licensed program.
