OnNovember1,1996,NIP-PRO-04,wasrevisedtoeliminatetheprovisionthatallowedimplementationofcertaindesignchangessuchassetpointsaseditorial.ThechangetoN2-IPM-SWP-R109hasbeenverifiedtobecorrect.NMPCacknowledgesthatthisisthesecondoccurrencewheretheNRChasidentifiedthattherequirementsoftheoperatinglicensewerenotadequatelytranslatedintoapprovedprocedures(seeIR50-220/96-01and50-410/96-01).However,itisnotedthattheNMPCQualityAssuranceDepartmentwasperforminganauditofoperationsatthetimethiseventoccurred.Theauditplanincludedthespecificintenttoaudittheprocedurechangeprocess.SincetheNRCidentifiedthisdeviationearlyintheaudit',NMPCcannotstatewithcertaintythatitwouldhaveotherwisebeendiscoveredduringtheaudit.Inanyevent,Deviation/EventReport(DER)C-96-1942waswrittenduringtheaudittodocumentinconsistenciesbetweenNDD-PROandNIP-PRO-04regardingeditorialchangestoprocedures.AsstatedinourresponsetoNoticeofViolation96-01-05,thefollowingcorrectiveactionshavebeentakenwithregardtotheeventdescribedinthatviolation.AreviewofthecontrolsdescribedinNDD-PRO,"ProceduresandOrders"andNIP-PRO-02,"PreparationandReviewofAdministrativeProcedures"hasbeencompleted.~AreviewofhighertierNuclearDivisionProcedures(NuclearDivisionDirectives(NDDs))forcompliancewiththelicensebaseshasbeencompleted.ThesecorrectiveactionswereexpandedbyNMPCtoincludeselectedNuclearInterfaceProcedures(NIPs)andGenerationAdministrativeProcedures(GAPs).Baseduponthatreview,NMPCplanstocompletelyreviewtheremainingNIPsandGAPsbyDecember31,1997.Thisreviewwillincludetheuseofenhancedcomputersearchcapabilitieswhichhaverecentlybeendevelopedforusebyprocedurewritersandbenchmarkingofotherlicensees'rocedureprograms.Anysignificantdiscrepanciesidentifiedduringthesereviewactivitieswillbedocumentedinourcorrectiveactionprogramforevaluationandcorrection.DuringthereviewofNDDs,NIPs,andGAPs,NiagaraMohawkhasdeterminedthatourreviewandapprovalprocessforadministrativeproceduresisnotasrigorousastheprocessfortechnicalprocedures.Therefore,wewillreviseasnecessarytheadministrativeprocedure~reviewandapprovalprocessbyApril30,1997.NMPCachievedfullcomplianceonNovember1,1996,whenNIP-PRO-04,Revision5,wasmadeeffectiv B.Unit1TechnicalSpecifications,Section6.8.1,requiresthatwrittenproceduresbeestablishedandimplemented.TheUnit2TechnicalSpecifications,Section6.8.1,alsorequiresthatwrittenproceduresshallbeestablishedandimplementedthatmeettherequirementsandrecommendationsofANSIN18.1-1972andAppendix"A"ofRegulatoryGuide1.33,whichcoversmaintenanceandsurveillanceactivities.NineMilePointUnit1surveillancetestNl-ST-Q1B,"CoreSprayLoop12PumpsandValveOperability,"Revision5,requiresthatdatafromstep8.3.21.dtobecarriedforwardtostep8.3.21.ffordeterminingcorespraytoppingpump121differentialpressure.Contrarytotheabove,onJune6,1996,duringtheperformanceofNl-ST-Q1B,datafromsteps8.3.21.dwasnotproperlycarriedforwardtostep8.3.21.f,resultinginNMPCbeingunawareuntilJuly17,1996,thatcorespraytoppingpump121exceededthesurveillancetestacceptancecriteriaforhighdifferentialpressure,2.NMPCprocedureGAP-OPS-02,"ControlofHazardousEnergyandConfigurationTagging,"Revision6,requiredthatduringtheapplicationofamarkup,theassignedoperatorplacedallnecessarydevicesintherequiredpositionandappliedacompletedtag.Contrarytotheabove,onJuly26,1996,duringtheapplicationofaUnit1markupforhydraulicunit(HCU)18-31,theassignedoperatorremovedthefusesfromHCU38-18,causingcontrolrod38-18toscram.3.NMPCprocedureGAP-PSH-01,"WorkControl,"Revision15,Section3.11.1,statesthatforchangesthatadverselyaffectthescopeorplantimpactstatementofaworkorder(WO)inprogress,workshallnotcontinueuntiltheWOhasbeenupdatedoranotherWOgenerated.Contrarytotheabove,onJuly25,1996,changesweremadetoaUnit2workorder(WO96-10638-00)relatedtotheDivisionIIhydrogen/oxygen(H,O)monitor.Thisadverselyaffectedtheplantimpactstatement,andworkproceededwithouttheWObeingupdatedoranotherWOgenerated.ThisresultedinI&CtechniciansworkingonthewrongdivisionoftheH,O,monitoringsystem.Inaggregate,thisisaSeverityLevelIVviolation(Supplement1).

ThethreeeventsdescribedintheviolationhadpreviouslybeenidentifiedinNMPC'scorrectiveactionprogram(NIP-ECA-01,Deviation/EventReport),Theprimarycauseofthethreecitedexamplesispersonnelerrorinthatappropriateself-checkingwasnotappliedpriortotheinitiationofanaction.Inthefirstexample,thefailuretocarryforwarddatafromsteps8.3.21.dtostep8.3.21.fofNl-ST-Q1B,wascausedbytheoperatorwhoperformedthecalculationandomittedthepressurecorrectionfortheevaluationofapressuregauge.Subsequently,theStationShiftSupervisor(SSS)discoveredtheinitialerrorandcorrectedaportionofthecalculation.However,theSSSfailedtocorrectthesubsequentcalculationsrequiredintheprocedureafterhavingdiscoveredtheerror.~Inthesecondexample,theinadvertentscramofcontrolrod38-31,theoperatormarkingupthehydrauliccontrolunitformaintenancefailedtoselfverifythathewaspullingthecorrectfuses.Thatishefailedtoassurethatthecorrectactionwastobetakenpriortoremovingthefuse.Further,hefailedtoensurethatapeerverificationwasperformedinaccordancewithmanagementexpectations.Inthethirdexample,thesupervisorwhopreparedachangetoWO96-10638-00failedtoperformadequateself-checkingwhenthechangewasmadetoensurethatitwasontheproperdivision.FollowingarethecorrectiveactionswhichhavebeentakenasdocumentedintheassociatedDERs.LicenseeEventReport96-06documentsthiseventandasnotedinInspectionReport96-10hasbeenclosedbytheNRC.ThecorrectiveactionsforLER96-06encompassthecorrectiveactionnecessarytorespondtothisviolationofTechnicalSpecificationsandareasfollows.1.OnJuly17,1996,surveillanceprocedureNl-ST-Q1Bwasre-performedforthecorespraytoppingpump121withacceptableresultsandthepumpwasdeclaredoperable.2.TheManagerofOperationscounseledtheindividualsinvolvedintheeventregardingtheimportanceofattentiontodetailandself-checking.TheManagerofOperationsdiscussedtheeventwitheachoperatingshift,focusingonself-checkingtechniquesandthenecessitytoensurecalculationsareperformedcorrectly.Also,theexpectationsforsupervisoryreviewoftestingactivitieswerereaffirme .TheProcedureWriter'sGuidegoverningOperation'sSurveillanceProcedureshasbeenrevisedtoenhancetheformatofindependentverificationstepsforcalculations.1.ImmediatecorrectiveactionsweretoreplacethefusesonHCU38-31andtocompletethemarkuponHCU18-31.2.ReactorEngineeringevaluatedandapprovedrestorationofcontrolrod38-31tothefulloutposition48position.3.OnJuly26,1996,instructionswereissueddirectingShiftSupervisorstoreviewtheOperationsReferenceManualregardingverificationactionspriortomarkingupCRDHCUsformaintenance.Particularemphasiswasplacedonexpectationsforconcurrent(peer)verifications.TheShiftSupervisorsweredirectedtocommunicateandenforcetheseexpectations.4.Jobperformancecounselingwascompletedfortheoperatorwhoperformedthemarkup.5.Shiftmanagementperformedaself-assessmentoftheresponsibleoperator'suseofself-verificationtechniques.TheManagerofOperationsreviewedthiseventwitheachoperatingshift,discussingthedifferentself-checkingtechniquesandwheneachisappropriate.>Q>~&r1.Theliftedleadswereimmediatelyrelandedandverified.2.Thesupervisorinvolvedwascounseledbymaintenancemanagement.3.TheUnit2MaintenanceManagerleadastanddownofMaintenancesupervisorsinwhichhecoveredpoorperformancebymaintenancewithregardtopersonnelerrors.Heemphasizedaccountabilityforindividualperformance.NiagaraMohawkcontinuallymonitorsperformancewithregardtopersonnelerrors.TheQuarterlyDERProgramTrendReportSummaryisutilizedtomonitortrendsonabroadbasis.EachBranchManagerisresponsibletoevaluatenegativeperformancetrendsandtakeappropriateactiontoreversethetrend.Inresponsetothespecificexamplescited,NiagaraMohawkmanagementcounseledtheindividualsinvolvedandrevieweddetailsofthespecificeventswithexpectationstothesupervisorypersonnelintheinvolvedbranches.(OperationsUnit1andMaintenanceUnit2)

TofurtherheightentheawarenessoftheimpactofpersonnelerrorsintheOperationsandMaintenancebranchesatbothunits,theBranchManagerswillreviewthisviolationwiththeirpersonnelbyFebruary28,1997.Severalactionshavebeenaddedtothe1997-2000SBUBusinessPlantofacilitatecontinuedimprovementregardinghumanperformanceatNineMilePoint.Objective4,StrategyEspecificallyaddresseshumanerrorsandimprovementinpersonnelperformance.Thestrategyaddressesmonitoringhumanperformance,monitoringtheself-assessmentprocess,improvingradiationworkerpractices,andstandardizingthecausedeterminationprocess.Fullcompliancewasachievedasfollows.ProcedureNl-ST-Q1BwasreperformedonJuly17,1996,withfullysatisfactoryresults.2.3.OnJuly26,1996,thefuseswerereplacedoncontrolrodHCU38-31.OnJuly25,1996,thesubjectliftedleadswererelandedandverifie ~i
