Routine unannounced inspection by the regional based inspector of work activities including welding, quality cssurance, quality control, work planning, receiving inspection, nondestructive testing, pro-cedures relative to reactor vessel safe end replacement and replacement of the

28" diameter recirculation system piping. The inspection included 30 inspector hours on site.
Results: One violation was identified in the seven areas inspected.
(Failure to correctly identify the weld seam examined on the radiograph).

Licensee Action on Previous Inspection Findings (Closed) Inspector Followup Item (220/82-13-02).
The inspector reviewed the current (Rev C1) list of qualified welders which indicates those welders qualified for automatic gas tungsten arc welding (AGTAW) using the video remote camera system. The selection of welders and appraisal of the need for additional training just prior to component welding is under control of the GAPC0 Welding Superintendent. The list of qualified w21ders provides for extension of each welder qualification based on use of the process and Q.A. verification.

(Closed) Unresolved Item (220/82-16-02). Weld Shrinkage Control. The inspector reviewed the Controlled Work Instruction (CWI) for replacement of the #13 suction pipe loop and provisions for weld shrinkage control.
The method to control weld shrinkage as shown in the CWI is the result of joint engineering review by NNI and NMPC.

(0 pen) Unresolved Item (220/82-16-01). Weld Wire Control.
The Weld Wire Issue Procedure (SI-QA-8) has been revised to require the supervisor of the welder to sign the weld issue chit.
Review of chits showed that they are being signed by the preparer. This item remains unresolved pending licensee review of chits issued before August 26, 1982 to assure they were prepared by the authorized individual.

Reactor Vessel Nozzle Safe End and Recirculation Piping Replacement a.
Work in Progress The inspector observed pipe / elbow machining, NDE, joint preparation, handling and welding in the drywell and at offsite fabrication areas.
The Controlled Work I.nstructions (CWI), signoffs by supervision and inspectors were reviewed.
No violations were noted.
b.
NMPC - QA/QC program - Control of Contractors and suppliers.
The inspector reviewed purchase orders for pipe, elbows and tees for the suction and discharge 28" piping for conformance to NUREG 0313 Revision 1.
The NMPC inspection program at suppliers shops and the available receiving reports for material on site were reviewed.
The program and status of inspection of NNI procedures and work by NMPC, QC were reviewed for conformance to applicable codes, standards and procedures.
No violations were noted.

Distribution of revised procedures or instructions and voiding of superseded pages.
The inspector noted that the controlled copy of the Weld Wire Issue procedure (SI-QA-8) in use at the NNI weld wire issue station was not the latest revision. This item is unresolved pending results of the NNI QA Audit of all controlled NNI procedures and instructions to determine the effectiveness of the distribution of Revisions.

Radiographic Examination of Welds The inspector reviewed a sample of pipe weld radiographs and reports for conformance to the ASME Code, contra-tor procedures and regu-latory requirements.
The radiographs for FW 12S-SEN had been intre-preted and reviewed by the radiographic subcontractor, the instal-lation contractor and the licensees quality assurance radiographer prior to the inspectors review. Three intervals (14-28, 28-42 and 42-56) of the final radiographs for FW 12S-SEN were found to be identified as those for FW11S-SEN. This is a violation of 10 CFR 50, Appendix B Criterion IX, for " Control of Special Processes".

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, violations or deviations. An unresolved item disclosed during the inspection is discussed in paragraph 3C.

Exit Interview An exit interview was held on October 29, 1982 with members of the licensees staff, as denoted in paragraph 1.
