was completed, however, longitudinal cracks in the thermal

sleeve were identified by radiographic examination. The licensee plans to replace the thermal sleeve. Various thermal sl.ceve designs are being studied by the licensee.

Ultrasonic examination was performed on the thermal sleeve on Loop B and no indication was found.

Flaw indication on the safety valve nozzle to steam drum weld is being evaluated by the licensee. The licensee is performing ultrasonic examination on the other nine (9) safety valve nozzle to steam drum welds.

Status of Previously Reported Unresolved Items 1.

Tapes Used on Stainless Steel Surfaces (IE Inspection Report No. 050-010/75-15)

During the previous inspection, it was determined that documentary evidence to show that tapes used on sur-faces of stainless steel have been evaluated and found acceptable was unavailable at the site.

I During this inspection, the inspector was informed that the tape had been removed from the six-inch stainless steel elbow and the elbow was cleaned with penetrant l

cleaner. The licensee also stated that QC procedure was revised to include the proper instructions and is now in printing. The inspector stated that this matter remains unresolved, pending review of QC procedure during a subsequent inspection.

Results of Inspection 1.

The licensee stated that the flaw indication on the safety valve nozzle to steam drum weld is being evaluated using the 1974 edition of the ASME Boiler and Pressure Vessel (B&PV)

'

Code Section XI.

The UT mapping used in this evaluation is based on the 1971 edition of the ASME B&PV Code Section XI.

In order to be consistent with the 1974 code, the licensee is obtaining a new UT test block and plans to map the flaw indication in accordance to the 1974 code. Final evaluation will then be performed based on the new mapping and the 1974 ASME B&PV Code Section XI criteria.

In addition to the code requirement, the licensee plans to perform a finite element analysis. The licensee also stated that, if the flow indic-ation is determined to be unacceptable the repairs will be performed in accordance with welding Procedure No. 3 or No. 4, of the 1974 ASME B&PV Code Section XI, with the exception that if Procedure No. 3 is used. heat treatment required by the 1974 code may not be fully met.

The inspector stated that NRC plans to review the evaluation in depth. The inspector also stated that even if the flaw indication-5-

is determined to be acceptable, this matter still has the possibility of beir g treated as an unreviewed safety -

matter as defined in 10 CFR Part 50, Paragraph 50.59.
