Topics Discussed

On June 4, 1975, members of Region III staff met with Commonwealth Edison (CE) personnel at the CE Corporate Office.

The purpose of the meeting was to discuss recent abnormal occurrences at the Commonwealth Edison operating nuclear reactor sites.

Each of the project inspectors related several events which occurred at the plants for which he has inspection responsibility, focusing on causes which involved:

(1) Operator error.

(2) Management control problems.

A discussion ensued by both NRC and CE personnel as to potential

,

j causes for these failures.

Some of the corrective actions discussed by CE were:

(1) Steps are being taken at each site to increase job knowledge through training.

Each site is expanding both training material and training personnel. A special first line supervisor course will be given at all CE sites with the first course being given at Dresden in August 1975.

(2) Methods of dealing with operator errors due to poor attitude or aptitude are being reviewed.

(3) Reorganization of the site management structure is

being reviewed.

(4) Each station is going through a major program of revising plant and administrative procedures to improve personnel knowledge and management control.

s At the conclusion of the meeting, NGC personnel again emphasized their concern relative to operator errors and management control.

The augmented inspection program to be instituted at the Dresden site was discussed.

It is estimated that this program would double the present inspection effort and would concentrate on areas discussed during the present meeting.

Topics Discussed A followup meeting with CE corporate management was held on June 24, 1975, at the CE corporate offices. The topics discussed during the meeting included:

(1) Recent operating problems at the Dresden, Quad-Cities, and Zion nuclear facilities.

(2) Commonality of factors contributing to the operating

(3) Management involvement, and overview of plant operations.

(4) NRC enforcement options.

(5) Augmented inspection program at the Dresden site.

(6) Future communications regarding NRC inspection findings.
