| Description | Technical Specification 3.8.1, AC Sources Operating, Action A.1, required that when one offsite source is inoperable, the licensee shall verify the remaining offsite source is operable within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and every eight hours thereafter. Contrary to the above, on July 4, 2009, the offsite power source to bus NB01 was inoperable and action was not taken within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> to verify the other offsite source was operable. This finding was entered in the licensees corrective action program as Callaway Action Request 200905373. This finding is greater than minor because it was associated with the Mitigating Systems Cornerstone attribute of human performance and it affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Manual Chapter 0609.04, Phase 1 Initial Screening and Characterization of Findings, the issue screened as very low safety significance because it was not a design or qualification deficiency that resulted in a loss of operability or functionality, did not create a loss of system safety function of a single train for greater than the technical specification allowed outage time and did not affect seismic, flooding, or severe weather initiating events |
