| Description | The inspectors identified an apparen
violation involving the failure to follow the corrective action procedure and take timely corrective action for a condition adverse to quality, in that the licensee failed to implement a modification to permit complete inspection and cleaning of the reactor vessel head and CRDM nozzles. This finding is more than minor because the corrosion of the reactor head and the resulting cavity represented a significant loss of the design basis barrier integrity. The significance of this finding will be determined by the Significance Determination Process for the issue, which was begun following the Augmented Inspection Team activities |
