| Title | Failure to Monitor the Non-interruptible Instrument Air Check Valves |
| Description | The NRC identified a Green non-cited violation of 10 CFR 50.65, Requirements for Monitoring the Effectiveness of Maintenance at Nuclear Power Plants, for the licensees failure to monitor the non-interruptible essential instrument air check valves in a manner sufficient to provide reasonable assurance that the components were capable of fulfilling their intended function. The licensee initiated CR 2009105109 and established compensatory measures to mitigate potential back leakage via the check valves during a loss of instrument air event. The team determined that the licensees failure to perform periodic maintenance on non-interruptible essential instrument air header check valves was a performance deficiency. This finding is more than minor because it is similar to example 7.d. of Inspection Manual Chapter 0612, Appendix E, and because it is associated with the equipment performance attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the failure to perform periodic maintenance or testing of the check valves resulted in a lack of reasonable assurance that the non-interruptible essential air system would provide sufficient capability to operate the hardened containment vent during a loss of instrument air event. The failure to operate the containment hardened vent could adversely affect the mitigation function of the decay heat removal system. This finding was assessed using the Phase 1 screening worksheet of the SDP and determined a Phase 3 analysis was required. Phase 3 results characterized the performance deficiency as very low safety significance (Green) based on risk.
