No. 2A Diesel Generator - Prelubrication Oil Heater - Inoperable.

This report was initiated by Incident Report 81-500.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 34% - Unit Load 320 MWe DESCRIPTION OF OCCURRENCE:

On December 15, 1981 the Control Operator received the DIESEL GENERATOR 2A URGENT TROUBLE alarm.

Upon investigation it was discovered that the prelubrication oil pump pressure was read-ing zero while the pump was running, the lubricating oil temperature was low, and the PRELUBE OIL PUMP FAILURE alarm was activated.

At 0620 hours0.00718 days <br />0.172 hours <br />0.00103 weeks <br />2.3591e-4 months <br /> 2A diesel was declared inoperable and Action Statement 3.8.1.1.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The prelubrication oil heater failed, causing the oil temperature to be low and viscosity high, preventing it from flowing properly.

Therefore, the pump, although running, could create no oil pressure.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.8.1.1.a requires:

l With either an offsite circuit or diesel generator of the above required A.C.

electrical power sources inoperable, demonstrate the operability of the remaining A.C.

CORRECTIVE ACTION

Surveillance Requirements 4. 8.1.1.1. a and 4. 8.1.1. 2.a. 2 were performed within one hour and every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> as required by Action Statement 3.8.1.1.a.

At 0320 hours0.0037 days <br />0.0889 hours <br />5.291005e-4 weeks <br />1.2176e-4 months <br />, December 16, 1981, 2A diesel was run to bring oil pressure and flow to normal.

At 2245 hours0.026 days <br />0.624 hours <br />0.00371 weeks <br />8.542225e-4 months <br />, December 17, 1981, it was determined that the diesel would be run periodically to maintain lubricating oil temperature above 1100F until a new heater could be procured and installed.

2A Diesel was declared operable and Action Statement 3.8.1.1.a was terminated.

Subsequently, on December 18, 1981, a new prelubrication oil heater was in-stalled, and 2A Diesel was tested satisfactorily.
