Containment Fan Coil Unit - Service Water Leak.

This report was initiated by Incident Report 81-480.

CONDITIONS PRIOR TO OCCURRENCE:

M?de 1 - Rx Power 50% - Unit Load 560 MWe DESCRIPTION OF OCCURRENCE:

On November 25, 1981, during a routine containment inspection, an operator discovered service water leaking from the second from top primary cooling coil, on No. 24 Containment Fan Coil Unit (CFCU),

at a rate of approximately 0.5 gallons per minute.

The CFCU was isolated by closing the inlet and outlet valves from the control room, but remained operable and capable of functioning adequately under emergency conditions.

In accordance with NRC IE Bulletin 80-24, the NRC was notified of the service water leak in contain-ment by telephone, with written confirmation transmitted within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

No. 24 CFCU was declared inoperable and Action Statement 3.6.2.3.a was entered at 0343 hours0.00397 days <br />0.0953 hours <br />5.671296e-4 weeks <br />1.305115e-4 months <br />, November 27, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The service water leak was due to a pinhole leak in the Lecond from top primary coil on No. 24 CFCU.

ANALYSIS OF OCCURRENCE:

Technical Specification of 3.6.2.3.a requires:

With one group of containment coolit.g fans inoperable and both Containment Spray Systems operable, restore the inoperable group of fans to operable st<.'us within 7 days or be in hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

I LER 81-ll5/OlT CORRECTIVE ACTION:

The second from top primary coil on No. 24 CFCU was satisfac-torily repaired with Belzona metal filler.

It was tested satisfactorily and returned to service.

At 1738 hours0.0201 days <br />0.483 hours <br />0.00287 weeks <br />6.61309e-4 months <br />, November 28, 1981, No. 24 CFCU was declared operable and Action Statement 3.6.2. 3.a was terminated.

A design change request has been submitted to replace this cooler during the first refueling outage; however, no supplementary report will be issued.
