Containment Fan Coil Unit - Inoperable - Low Service Water Flow.

This report was initiated by incident report 81-379.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 90% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On September 18, 1981 the control room operator noticed low service water flow indication on No. 24 Containment Fan Coil Unit (CFCU).

No. 24 CFCU was declared inoperable and action statement 3.6.2.3.a was entered at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the service water low flow indication was the flow transmitter was plugged with silt.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both containment spray systems operable, restore the inoperable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The flow transmitter sensing lines were blown down to remove the silt.

The transmitter was tested satisfactorily and returned to service.

At 0940 hours0.0109 days <br />0.261 hours <br />0.00155 weeks <br />3.5767e-4 months <br />, September 18, 1981 action statement 3.6.2.3.a was terminated.

A program has been instituted to blow down all transmitters every week.
