No. 21 Steam Flow Channels 1 and 2 - Inoperable This report was initiated by Incident Report 81-362.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On September 3, 1981, after an engineering review, it was decided that No. 21 st am flow channels 1 and 2 were both out of calibra-tion due to excessive carryover from No. 21 steam generator.

At 1610 hours0.0186 days <br />0.447 hours <br />0.00266 weeks <br />6.12605e-4 months <br />, September 3, 1981, both steam flow chant.els were declared inoperable and actions were initiated in accordance with applicability statement 3.0.3.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Due to excessive carryover from No. 21 steam generator, No. 21 steam

flow channels 1 and 2 were determined to be out of calibration.

ANALYSIS OF OCCURRENCE:

Applicability statement 3.0.3 requires:

When a limiting condition for operation is not met except as provided in the associated action requirements, within one hour action shall be initiated to place the unit in a mode in which the specification does not apply by placing it, as applicable, in:

At 2015, September 3, 1981, applicability statement 3.0.3 was terminated and action statement 3.3.2, action 14 was entered.

At 2200, September 3, 1981, No. 21 steam flow channel 2 transmitter was recalibrated and returned to service, and action statement 3.3.2, action 14 was terminated.
