IDENTIFICATION OF OCCURRENCE:

Containment Fan Coil Unit - Service Water Leak.

l This report was initiated by Incident Reports81-307 and 81-312.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 75% - Unit Load 780 MWe DESCRIPTION OF CCCURRENCE:

On August 6, 1981, during a routine containment inspection, an operator discovered a service water leak of approximately.5 gallons per minute emanating from the body to bonnet sealing area of valve 22SW65 on No. 22 containment fan coil unit (CFCU).

In accordance with NRC IE Bulletin 80-24, the NRC was notified of the service water leak by telephone with prompt notification made to the resident NRC Inspector at 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br /> on August 7, 1981.

No. 22 CFCU was removed from service and the service water leak was isolated.

Action state-ment 3.6.2.3.a was entered at 2250 hours0.026 days <br />0.625 hours <br />0.00372 weeks <br />8.56125e-4 months <br />, August 6, 1981, after

]

declaring the CFCU inoperable due to service water isolation.

4 On August 7, 1931, while preparing the CFCU for testing after the repair of 22r.55 the tell-tale leakage line from the CFCU motor cooler indicated a leak rate of approximately 1.6 gallons per minute.

The leakage was determined to come from a weld joint next to the service water inlet flange.

Service water to the CFCU was isolated and the CFCU was again declared inoperable.

Action statement 3.6.2.3.a remained in effect.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

CORRECTIVE ACTION

i The body to bonnet studs on valve 22SW65 were tightened and the leak stopped.

The weld ~ joint on the CFCU motor cooler was weld repaired and hydrostatically tested with satisfactory results.

The CFCU was tested satisfactorily and returned to service.

At 2028 4

hours on August 10, 1981, action statement 3.6.2.3.a was terminated.
