Individual Rod Position Indicators - ISB2 and 2SB2 - Inoperable.

This report was. initiated by Incident Reports81-246 and 81-247.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 3% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

At 0855.and 1042 hours0.0121 days <br />0.289 hours <br />0.00172 weeks <br />3.96481e-4 months <br />, respectively, on July 13, 1981, the control.

operator noticed that the individual red position indicators for rods'lSB2 and 2SB2 were in disagreement with the shutdown rod bank demand indicators by greater than + 12 steps.

The individual rod position indicators were declared Inoperable at said hours, and action statement 3.1.3.2.1.a was entered.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The individual rod position indicators for rods ISB2 and 2SB2 were not in agreement with the shutdown rod bank demand indicators due to failure of the operational amplifiers on the amplifier boards in the indicators.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.1.3.2.1 requires that in modes 1 and 2 with a maximum of one rod position indicator per bank inoperable eicher:

Determine the position of the non-indicating rod (s) indirectly by the movable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's

'In the position indicator for rod lSE2, the amplifier board was replaced..

In the position indicator-for rod 2SB2, the operational amplifier on the amplifier board was replaced.

The position indicators for rods ISB2 and 2SB2 were recalibrated, tested. satis-factorily, and returned to service.
