This report was initiated by Incident Report 81-258.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 20% - Unit Load 200 MWe l

DESCRIPTION OF OCCURRENCE:

On July 16, 1981, the control operator received a low flow alarm on the service water f3ow of No. 21 containment fan coil unit (CFCU).

No. 21 CFCU was dociared inoperable and action statement 3.6.2.3.a was entered at 0550 hours0.00637 days <br />0.153 hours <br />9.093915e-4 weeks <br />2.09275e-4 months <br /> on July 16, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The inlet transmitter 2FA-3160Z-1 of No. 21 CFCU was clogged with silt.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

Transmitter 2FA-3160Z-1 was removed from service, and both legs were blown down.

The equalizer valve was opened, and the blowdown was continued until clean water came out.

Both vent valves were flushed.

The transmitter was returned to service and tested satisfactorily.

Action statement 3.5.2.3.a was terminated at 1456 hours0.0169 days <br />0.404 hours <br />0.00241 weeks <br />5.54008e-4 months <br /> on July 16, 1981.

A program is currently-being planned to blow down all transmitters at regular intervals.
