Service Water Leakage in Containment.

This report was initiated by Incident Report 81-206.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - 'Mit Load 0 MWe DESCRIPTION OF OCCURRL E:

With the Unit in Hot Standby conditions, during the performance of Startup Procedure (SUP 82.6), Loss of Offsite Power, a service water leak in containmenc was suspected due to the continuous running of the Containment Sump Pumps.

An investigation was started to determine the source of the suspected service water leak.

The investigation revealed that the threaded pipe cap on the motor cooler vent 11ae for No. 21 Containment Fan Coil Unit had backed off.

At 1133 hours0.0131 days <br />0.315 hours <br />0.00187 weeks <br />4.311065e-4 months <br />, June 23, 1981, the service water to No. 21 Containment Fan Coil Unit was isolated and Action Statement 3.F.2.3.a was entered.

Approximately 12,000 ga'lons of service water leaked into the containment before the leak was isolated.

This leakage was pumped to the Liquid Waste System waste hold up tanks by the Containment Sump Pumps.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9 b. and is also reportable in j

accordance with I.E.

Bulletin 80-24 dated November 21, 1980, for

" Prevention of Damage Due to Water Leakage Inside Containment."

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

l Equipment failure.

Vibration had caused the 1/2 inch threaded pipe cap to back off the vent line on No. 21 Containment Fan Coil Unit motor cooler.

ANALYSIS OF OCCURRENCE:

CORRECTIVE ACTION

The threaded pipe cap was inspected and re-tightened'.

The Containment Fan Coil unit was made ready, returned to service and inspected for service water leakage.

No leakage was evident.
