Title | Inadequate Corrective Actions to Promptly Correct Leaking Snubber Valves on the A Emergency Diesel Generator |
Description | The NRC identified a Green non-cited violation of 10 CFR 50, Appendix B, Criterion XVI, Corrective Action, for Entergy\'s failure to promptly correct a condition adverse to quality. Specifically, Entergy did not correct defective material in their A Emergency Diesel Generators (EDG) in a prompt manner which led to emergent maintenance and additional unplanned unavailability of the A EDG while they replaced cracked snubber valves. Entergy\'s corrective actions include entering this issue into the corrective action program and replacing the seven remaining snubber valves on their AEDG with those of a material properly hardened and not susceptible to the same mode of cracking. The inspectors determined that the finding was more than minor because the finding was associated with the Equipment Performance attribute of the Mitigating Systems Cornerstone, and adversely affected the cornerstone\'s objective to ensure the availability of systems that respond to initiating events to prevent undesirable consequences (i.e., core damage). Specifically, the A EDG was unavailable during snubber valve replacements. The inspectors determined the significance of the finding using IMC 0609.04, Phase 1 -Initial Screening and Characterization of Findings. The finding was determined to be of very low safety significance (Green) because the finding did not result in a loss of system safety function of a single train for greater than its Technical Specifications outage time, and did not screen as potentially risk significant due to external initiating events. This finding had a cross-cutting aspect in the area of Problem Identification and Resolution, Corrective Action Program component, because Entergy did not take corrective actions in a timely manner. Specifically, Pilgrim did not replace the A EDG snubber valves in a prompt manner after repeated fuel leaks from cracked snubber valves over the previous two years. |
