Breach of Fire Barrier Between Turbine Building and Ventilation and Equipment Room.

Conditions Prior to Occurrence:

100% FP Description of Occurrence: At approximately 0950 hours0.011 days <br />0.264 hours <br />0.00157 weeks <br />3.61475e-4 months <br /> on April 22, 1981, during a seismic block wall inspection, a breach of the fire barriar between the Unit 3 Turbine Building and the Ventilation and Equipment Room in penetration 3-T-T-1 was discovered.

The breach measured 10" deep, with a 2" opening at one end and increasing to a 6" opening at the other. A fire watch was established at approxi-mately 1000.

This constitutes operation in a degraded mode per Technical Speci-fication 3.17.5 and is thus reportable pursuant to Technical Specification 6.6.2.1.b(2).

Apparent Cause of Cccurrence:

The cause of the breach was due to the pulling of electrical cables through the barrier to supply the Unit 3 Turbine Building Officit Analysis of Occurrence:

The breaching of this fire barrier. could have possibly allowed the spreading of fire from the turbine building to the ventilation and equipment room.

The void was of significant size to allow flames and/or smoke to pass through the barrier.

Because of the low combustibility of materials in the area, there was not an immediate danger to safety related or any other equip-ment.

After the discovery of the breach the required fire watch was maintained until the breach was repaired.

Hence the health and safety of the public was not endangered.
